摘要
目的探讨凝血酶冻干粉联合奥美拉唑治疗消化道溃疡出血的临床效果及纤维蛋白原变化。方法根据随机数字表法,将我院2017年1月~2018年2月90例消化道溃疡出血患者,随机分成2组。对照组给予奥美拉唑治疗(n=45),观察组则给予奥美拉唑联合凝血酶冻干粉治疗(n=45)。比较两组消化道溃疡出血疗效;消化道溃疡出血停止的时间、消化道溃疡愈合的时间、幽门螺杆菌转阴的时间;治疗前后患者纤维蛋白原指标和胃液酸碱度;用药安全特点。结果观察组消化道溃疡出血疗效明显优于对照组(P<0.05);与对照组相比,观察组消化道溃疡出血停止的时间、消化道溃疡愈合的时间、HP转阴的时间明显缩短(P<0.05);治疗后观察组纤维蛋白原指标和胃液酸碱度明显优于对照组(P<0.05)。结论奥美拉唑联合凝血酶冻干粉治疗消化道溃疡出血的临床疗效明显优于单用奥美拉唑,并且能有效改善纤维蛋白原指标和胃液酸碱度,加速溃疡愈合及止血,用药安全,值得临床推广应用。
Objective To investigate the clinical effects and fibrinogen changes of thrombin lyophilized powder combined with omeprazole in the treatment of peptic ulcer hemorrhage.Methods According to the random number table,90 patients with gastrointestinal ulcer bleeding from January 2017 to February 2018 were divided into 2 groups.The control group was given omeprazole,and the observation group was given omeprazole combined with thrombin lyophilized powder.The efficacy of bleeding in peptic ulcers;the time of gastrointestinal ulcer bleeding stopped,the time of peptic ulcer healing,and the time of Helicobacter pylori(HP)negative;the fibrinogen index and gastric acidity of patients before and after treatment;and the safety of medication were compared between the two groups.Results The efficacy of peptic ulcer hemorrhage in the observation group was significantly better than that in the control group(P<0.05);the bleeding time of the peptic ulcer in the observation group,the healing time of the peptic ulcer,and the time of HP negative change were better in the observation group(P<0.05);After treatment,the fibrinogen levels in the observation group and pH in the gastric juice were better than those in the control group(P<0.05).There was no significant difference in safety characteristics between the observation group and the control group(P>0.05).Conclusion Omeprazole combined with thrombin lyophilized powder for the treatment of peptic ulcer hemorrhage has a definite curative effect.It can effectively improve the fibrinogen index and pH of gastric juice,accelerate the healing of ulcers,accelerate hemostasis,and is safe for use.It is worthy of popularization and application.
