KIM-1在肾缺血再灌注损伤小鼠及亚临床AKI患者中的表达

Expression of KIM-1 in mice after ischemia/reperfusion injury and subclinical AKI patients

摘要 目的观察肾损伤分子1(kidney injury molecule 1,KIM-1)在肾缺血再灌注损伤(ischemia/reperfusion injury,IRI)小鼠及亚临床AKI患者中的表达情况,探讨KIM-1能否成为肾小管损伤的早期检测指标。方法①动物实验:利用C57BL/6小鼠建立IRI模型(夹闭时间为20 min),分别在第0 h、3 h、6 h、12 h、24 h取血、留尿及留取肾组织标本,检测各组血肌酐、肾小管损伤程度、肾组织及尿液中KIM-1在各时间点的变化;②临床实验:选择肾病综合征符合微小病变(minimal change disease,MCD)的患者,分为单纯MCD和MCD合并急性肾小管损伤两组,所有入选患者血肌酐值都在正常范围,故后者又称为亚临床AKI组;观察两组病例肾组织及尿液中KIM-1的表达变化。结果小鼠IRI后早期,KIM-1在肾组织和尿液中表达上调,且血肌酐的升高晚于KIM-1的表达;在MCD患者中,亚临床AKI组肾组织及尿液中KIM-1的表达明显增加。结论KIM-1可能成为检测肾小管损伤的早期的非创伤的敏感的生物学标志物。 Objective To observe the expression of KIM-1 in mice after ischemia/reperfusion injury and subclinical AKI patients and investigate whether KIM-1 could be an early and sensitive biomarker of tubular injury.Methods①Animal experiment:we subjected C57 BL/6 mice to bilateral ischemia/reperfusion injury(IRI)of 20 min.Serum creatinine,tubular injury and KIM-1 expression in both kidney and urine were detected.②Clinical experiment:all enrolled minimal change disease(MCD)patients diagnosed by clinic manifestation and renal pathology were divided into two groups:MCD without tubular injury(MCD only)and MCD with tubular injury by renal biopsy.KIM-1 expression was detected in both kidney and urine.Results Renal and urinary Wnt4 expression increased significantly at 3 h and increased further at 24 h post IRI.Both renal and urinary KIM-1 expression were significantly increased in subclinilcal AKI patients.Conclusion KIM-1 could be a potential noninvasive biomarker for the early detection of tubular injury.
