某院耐碳青霉烯类肠杆菌科细菌的耐药基因分析

摘要 目的了解某院临床分离耐碳青霉烯类肠杆科细菌(CRE)的耐药基因分布,为抗感染治疗提供依据。方法收集某院2016—2019年临床分离的CRE菌株,用改良碳青霉烯类失活法(mCIM)及EDTA碳青霉烯类失活法(eCIM)分析耐药表型,PCR法检测碳青霉烯酶(CPAs)基因(blaKPC、blaGES、blaSME、blaIMP、blaNDM、blaVIM、blaOXA-48);分析主要菌种CAPs基因的占比差异。结果65株CRE中,mCIM和eCIM试验的阳性率分别为93.8%(61/65)和61.5%(40/65);共检出CPAs基因60株,其中blaNDM-1和blaNDM-5各19株,blaKPC-218株,blaIMP 3株,blaVIM-631株,未检出blaGES、blaSME及blaOXA-48。主要菌种CAPs基因占比差异有统计学意义。结论某院临床分离的CRE主要携带的CPAs基因是blaNDM-1、blaNDM-5及blaKPC-2;不同菌种主要携带的CPAs基因不全相同。
