催产素复合舒芬太尼镇痛泵静脉持续泵入对腹腔镜子宫肌瘤剔除术患者镇痛效果与术后焦虑的影响观察

Effect of continuous intravenous infusion of oxytocin combined with sufentanil analgesia pump on the analgesic effect and postoperative anxiety of patients undergoing laparoscopic myomectomy

目的 :探讨催产素复合舒芬太尼对腹腔镜子宫肌瘤剔除术患者的应用效果。方法 :选取2019年8月―2021年8月,宿州市第一人民医院收治的80例子宫肌瘤患者,随机分为观察组与对照组,各40例。对照组采取单纯舒芬太尼镇痛,观察组采取催产素复合舒芬太尼镇痛,比较两组患者镇痛效果、术中各时间点Ramsay评分、VAS评分和HAD评分,以及术后不良反应总发生率。结果 :观察组患者术后镇痛优良率为97.50%,显著高于对照组75.00%。两组患者术后1 h、48 h的Ramsay评分无明显差异,观察组术后12 h、术后24 h的Ramsay评分高于对照组。观察组术后12 h、24 h、48 h,VAS评分低于对照组。两组患者术后1 h的HAD评分无明显差异。观察组术后12 h、24 h、48 h,HAD评分显著低于对照组。观察组不良反应总发生率7.50%,显著低于对照组25.00%。结论 :腹腔镜子宫肌瘤剔除术中应用催产素复合舒芬太尼的镇痛镇静效果好,优于单用舒芬太尼,且可有效抑制炎症反应及抗焦虑,且随着舒芬太尼浓度的下降,不良反应减少。

Objective To investigate the effect of continuous intravenous infusion of oxytocin combined with sufentanil analgesia pump on the analgesic effect and postoperative anxiety of patients undergoing laparoscopic myomectomy.Methods 80 patients with uterine fibroids admitted to the first people's Hospital of Suzhou City from August 2019 to August 2021 were selected and randomly divided into observation group and control group,40 cases each.The control group was treated with sufentanil alone and the observation group was treated with oxytocin combined with sufentanil.The analgesic effect of the two groups,the Ramsay score,the VAS score and the HAD score at each time point,and the total incidence of postoperative adverse reactions were compared.Results The excellent rate of analgesia in the observation group was 97.50%,higher than that in the control group(75.00%).The Ramsay scores of the two groups were similar at 1 h and 48 h after operation,and the Ramsay scores of the observation group at 12 h and 24 h after operation were significantly higher than those of the control group.VAS score and had score in the observation group were lower than those in the control group at 12 h,24 h and 48 h after operation.The adverse reactions in control group(25.00%)was higher in the observation group(7.50%).Conclusion Oxytocin combined with sufentanil analgesia pump continuous pumping for postoperative analgesia in patients undergoing laparoscopic myomectomy can achieve good analgesia and sedative effect,superior to sufentanil alone,and can effectively inhibit Inflammatory response and anxiolytic,and with the decrease in sufentanil concentration,the adverse reactions are reduced.
