AMH联合FSH在腹腔镜下卵巢囊肿剥除术对卵巢储备功能影响的评价作用

Evaluation of the functional effect of AMH combined with FSH on laparoscopic ovarian cyst removal on ovarian reserve

目的:探讨AMH联合FSH在腹腔镜下卵巢囊肿剥除术对卵巢储备功能影响的评价作用。方法:选取2019年10月~2020年12月到我院因卵巢良性肿物行剔除术的80例患者作为研究对象。采取随机分组的方法,将患者分为腹腔镜组和开腹组,各40例。选取正常育龄妇女20例为正常对照组。比较各组间术前、术后激素水平及阴道超声指标,并进行Pearson相关性分析和预测价值评估。结果:术前,三组间血清AMH、E2,FSH,LH水平及AFC、RI、PI差异无统计学意义。术后1、3、6个月,腹腔镜组和开腹组的血清E2、LH、RI、PI与对照组比较无统计学差异,血清AMH、AFC均显著低于对照组,FSH显著高于对照组;术后1、3、6个月,腹腔镜组血清AMH、AFC均高于开腹组,FSH均显著低于开腹组。经过Pearson相关性分析,血清AMH与AFC呈正相关,与FSH呈负相关。AMH+FSH联合检测敏感度、特异度及准确度均显著高于单项指标检测结果。结论:腹腔镜下卵巢囊肿剥除术对卵巢储备功能有一定损伤,AMH联合FSH能有效对术后卵巢储备功能进行评价。

Objective To evaluate the effect of AMH and FSH on laparoscopic ovarian cyst removal on ovarian reserve function.Methods 80 patients who went to our hospital from October 2019 to December 2020 for benign ovarian swelling were selected as the study subjects.A randomization approach divided patients into laparoscopic and laparotomy,with 40 each.20 women of normal childbearing age were selected as normal control groups.The preoperative and postoperative hormone levels and vaginal ultrasound indicators were compared between the groups,and Pearson correlation analysis and predictive value assessment were performed.Results There was no significant difference in serum AMH,E2,FSH,LH levels and AFC,RI,PI between the three groups.1,3 and 6 months after surgery,there was no significant difference in serum E2,LH,RI,PI between laparoscopic and open groups,serum AMH,AFC significantly lower and FSH significantly higher than controls;1,3 and 6 months after surgery,serum AMH,AFC were higher in the laparoscopic group and FSH was significantly lower than the open group.After Pearson correlation analysis,serum AMH was positively correlated with AFC and negatively associated with FSH.The sensitivity,specificity and accuracy of AMH+FSH combination detection were all significantly higher than the detection results of single indicators.Conclusion Laparoscopic ovarian cyst removal damages the ovarian reserve function,and AMH and FSH can effectively evaluate the postoperative ovarian reserve function.
