白芥子穴位贴敷联合体表定位高频振动循经叩背治疗COPD急性加重期的安全性和有效性分析

Safety and effectiveness of acupoint application of white mustard seeds combined with body surface positioning and high frequency vibration followed by meridian percussion in the treatment of acute exacerbation of COPD

目的:探讨白芥子穴位贴敷联合体表定位高频振动循经叩背治疗慢性阻塞性肺病急性加重期(COPD)的安全性和有效性。方法:选择2020年1月~12月在我院接受治疗的30例COPD急性加重期患者为研究对象,按随机数字表随机分为试验组和对照组,每组各15例。试验组采用体表定位高频振动循经叩背排痰联合白芥子穴位贴敷。对照组采用传统的手拍方式进行拍背排痰。对两组患者的治疗效果、血气分析指标[pH值、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]、临床指标(机械通气时间、总排痰量、住ICU日数)以及炎性指标[血清白介素-6(IL-6)水平、血清白介素-8(IL-8)水平、肿瘤坏死因子-α(TNF-α)水平]进行比较。结果:与对照组相比,试验组患者治疗后pH值、PaO_(2)高且研究组PaCO_(2)低。与对照组相比,试验组患者机械通气时间、住ICU日数短且研究组总排痰量少。与对照组相比,试验组患者总有效率高。与对照组相比,试验组患者治疗后血清IL-6、IL-8、TNF-α水平低。结论:白芥子穴位贴敷联合体表定位高频振动循经叩背治疗能改善COPD急性加重期患者炎性指标、血气分析指标,缩短机械通气时间、住ICU日数,减少总排痰量。

Objective To explore the safety and effectiveness of acupoint application of white mustard seeds combined with body surface positioning and high frequency vibration followed by meridian percussion in the treatment of acute exacerbation of COPD.Methods A total of 30 patients with acute exacerbation of COPD treated in our hospital from January to December 2020 were selected as the research subjects and randomly divided into experimental group and control group according to random number table,with 15 patients in each group.In the experimental group,the body surface positioning high-frequency vibration was used to pass the meridian and tap the back to discharge sputum combined with acupoint application of white mustard seed.In the control group,the traditional hand patting method was used for sputum removal.Treatment effect of two groups of patients,blood gas analysis index[pH,arterial blood oxygen partial pressure,arterial blood(PaO2)co2 partial pressure(PaCO2)],clinical indicators(mechanical ventilation time and total amount of expectoration drainage,ICU days)and inflammatory index[serum interleukin 6(IL-6)level,serum level of interleukin 8(IL-8),tumor necrosis TNF-α levels were compared].Results Compared with the control group,pH and PaO2 in experimental group were higher after treatment,while PaCO2 in experimental group was lower.Compared with the control group,mechanical ventilation duration and ICU days in experimental group were shorter,and total sputum output in experimental group was less.Compared with the control group,the total effective rate in experimental group was higher.Compared with the control group,serum levels of IL-6,IL-8 and TNF-α in experimental group were lower after treatment.Conclusions In patients with acute exacerbation of COPD,acupoint application combined with body surface positioning and high-frequency vibration can improve inflammatory indexes,blood gas analysis indexes,shorten mechanical ventilation time,ICU days,and reduce total sputum output.
