潮气呼吸肺功能测定在百日咳的应用

Clinical application of tidal breathing and lung function measurement in children with pertussis

摘要 目的:探究潮气呼吸肺功能测定在百日咳患儿中的临床应用效果。方法:选取我院2018年12月~2020年1月收治的百日咳住院患儿63例作为百日咳组,同期选取来我院体检的同龄健康儿童68例作为健康组。于病情急性期、恢复期及随诊期各期分别对百日咳组进行潮气呼吸肺功能测定,健康组儿童仅进行一次测定,对比两组潮气呼吸肺功能参数结果。结果:急性期,百日咳组患儿RR高于健康组,TV/kg、Ti、Te、Ti/Te、TPTEF/TE、VPTEF/VE均显著低于健康组。百日咳组患儿恢复期的RR低于急性期,但高于健康组,TV/kg、Ti、Te、Ti/Te、TPTEF/TE、VPTEF/VE均显著高于急性期,但低于健康组。出院后3个月、6个月后,百日咳组的RR、TV/kg、Ti、Te、Ti/Te、TPTEF/TE、VPTEF/VE与健康组均无显著差异。出院后6个月,两组下呼吸道感染率(3.17%、1.47%)比较无显著差异,和两组均无气促、喘息发生以及均无因肺部阻塞性疾病就诊或住院的患儿。结论:百日咳对患儿肺功能有近期影响,但无长期影响。潮气呼吸肺功能检测可为婴幼儿百日咳的临床诊断及治疗提供辅助依据。 Objective To explore the clinical application effect of tidal breathing lung function measurement in children with pertussis.Methods 63 hospitalized children with pertussis treated in our hospital from December 2018 to January 2020 were selected as the pertussis group,and 68 healthy children of the same age who came to our hospital for physical examination were selected as the health group in the same period.Tidal breathing and lung function were measured in the pertussis group during the acute phase,recovery phase and follow-up period.Children in the healthy group were measured only once,and the results of the tidal breathing and lung function parameters of the two groups were compared.Results In the acute phase,the RR of children in the pertussis group was higher than that in the healthy group,and TV/kg,Ti,Te,Ti/Te,TPTEF/TE,and VPTEF/VE were significantly lower than those in the healthy group.The RR of children in the pertussis group during the recovery period was lower than that of the acute period,but higher than that of the healthy group,and the TV/kg,Ti,Te,Ti/Te,TPTEF/TE,and VPTEF/VE were all significant higher than the acute stage,but lower than the healthy group.3 months and 6 months after discharge,there was no significant difference in RR,TV/kg,Ti,Te,Ti/Te,TPTEF/TE,VPTEF/VE between the pertussis group and the healthy group.6 months after discharge,there was no significant difference in the lower respiratory tract infection rate(3.17%,1.47%)between the two groups,and there was no children with shortness of breath,wheezing and hospitalized due to pulmonary obstructive diseases in the two groups.Conclusion Pertussis has short-term effects on children’s lung function,but no long-term effects,Tidal breathing and lung function testing can provide auxiliary basis for the clinical diagnosis and treatment of whooping cough in infants and young children.
