CEUS中病灶-肺组织造影剂到达时间在肺周围型病变性质鉴别中的应用

摘要 目的:研究超声造影(CEUS)中病灶-肺组织造影剂到达时间在肺周围型病变性质鉴别中的应用价值。方法:纳入2018年1月~2021年3月柳州市人民医院收治的86例肺周围型病变患者为研究对象,根据病理检查结果将患者分为良性肿瘤组(n=46)和恶性肿瘤组(n=40)。病理诊断前,患者均行CEUS检查,记录患者病灶-肺组织造影剂到达时间。比较良性肿瘤组和恶性肿瘤组造影剂到达病灶时间、到达肺组织时间、病灶-肺组织时间差,并采用ROC曲线评价CEUS造影剂到达病灶-肺组织时间对肺周围型病变性质的鉴别价值。结果:良性肿瘤组造影剂到达病灶时间及病灶-肺组织时间差显著少于恶性肿瘤组。ROC曲线分析显示,造影剂到达病灶时间、病灶-肺组织时间差鉴别肺周围型病变性质曲线下面积分别为0.824、0.875,造影剂到达病灶时间联合病灶-肺组织时间差鉴别肺周围型病变性质的曲线下面积为0.896,灵敏度为84.3%,特异度为72.1%。结论:观察CEUS造影剂到达病灶时间及病灶-肺组织造影剂到达时间差对鉴别肺周围型病变性质具有较高的准确性,值得临床推广。 Objective To study the application value of focus lung tissue contrast agent arrival time in the identification of peripheral pulmonary lesions in contrast-enhanced ultrasound(CEUS).Methods 86 patients with peripheral pulmonary diseases treated in Liuzhou people’s Hospital from January 2018 to March 2021 were included as the research objects,The patients were divided into benign tumor group(n=46)and malignant tumor group(n=40)according to the pathological examination results.Before pathological diagnosis,all patients underwent CEUS examination,and the arrival time of contrast medium in focus lung tissue was recorded.The time of arrival of CEUS contrast agent at the lesion,time of arrival at lung tissue and time difference between lesion and lung tissue were compared between benign tumor group and malignant tumor group,and the value of time of arrival of CEUS contrast agent at the lesion lung tissue in differentiating the nature of peripheral lung lesions was evaluated by ROC curve.Results The time difference between the arrival time of contrast medium and the time difference between focus and lung tissue in benign tumor group was significantly lower than that in malignant tumor group.ROC curve analysis showed that the area under the curve of contrast agent arrival time and focus lung tissue time difference in differentiating the nature of peripheral pulmonary lesions was 0.824 and 0.875 respectively.The area under the curve of contrast agent arrival time combined with focus lung tissue time difference in differentiating the nature of peripheral pulmonary lesions was 0.896,with a sensitivity of 84.3%and a specificity of 72.1%.Conclusion Observing the CEUS contrast agent arrival time and the arrival time difference of contrast medium between focus and lung tissue has high accuracy in identifying the nature of peripheral lung lesions,and it is worthy of clinical promotion.
