小儿经皮肾穿刺活组织病理检查的临床应用

Objective to investigate the clinical value,acceptability and safety of percutaneous renal biopsy in children

目的:探究小儿经皮肾穿刺活组织病理检查的临床应用。方法:对怀化市第一人民医院2019年3月~2020年5月19例患肾脏疾病患儿进行经皮肾活检术,将活检组织进行光镜、免疫荧光、电镜检查,统计患儿穿刺成功率及并发症。采用问卷调查形式了解患儿及患儿家属对经皮肾活检术的接受程度,并探讨影响患儿活检依从性的原因。结果:患儿穿刺成功率为100%,有原发性肾小球疾病8例,继发性肾小球疾病10例,孤立性血尿、蛋白尿1例。1例患儿出现一过性血尿。本研究共发出19份调查问卷,问卷回收率为100%。患儿及家属对肾穿刺活检术的知识信息均来自医护人员(84.21%),多数患儿对经皮肾活检检测结果感到满意(94.74%),患儿在检查过程中对操作困难(42.11%)、出现并发症(73.68%)等顾虑占比较高。结论:小儿经皮肾活检术成功率高,患儿接受度高,且不良反应较少,可有效诊断患儿疾病,并有效评估患儿预后疗效。

Objective To explore the clinical value,acceptability and safety of percutaneous renal biopsy in children.Methods From March 2019 to May 2020 in our hospital,19 cases of children with renal diseases were treated with percutaneous renal biopsy.The biopsy tissues were examined by light microscopy,immunofluorescence and electron microscopy,and the success rate of puncture and complications were counted.Objective to investigate the acceptance of percutaneous renal biopsy in children and their families by questionnaire,and to explore the reasons affecting the compliance of children with renal biopsy.Results The success rate of puncture was 100%.There were 8 cases of primary glomerular disease,10 cases of secondary glomerular disease,1 case of isolated hematuria and proteinuria.One patient had transient hematuria.A total of 19 questionnaires were sent out,and the recovery rate was 100%.Most of the patients were satisfied with the results of percutaneous renal biopsy(94.74%),and most of them were concerned about the difficulty of operation(42.11%)and the occurrence of complications(73.68%).Conclusion Percutaneous renal biopsy in children has high success rate,high acceptance and less adverse reactions,which can effectively diagnose the disease and effectively evaluate the prognosis of children.
