内镜下冷圈套器切除结肠息肉的临床疗效观察

Observation of the Clinical Effect of Endoscopic Cold Snare in Resection of Colonic Polyps

目的:对内镜下冷圈套器切除结肠息肉的临床疗效进行观察评价。方法:选取2017年1月~2018年11月长沙市第四医院经结肠镜+病理组织学确诊的结肠息肉(5mm≤息肉直径≤10mm)患者120例,随机均分为观察组(冷圈套器切除)和对照组(热圈套器切除),比较两组患者息肉治疗术中出血率、术后出血率、穿孔率、临床症状严重程度积分、住院天数及费用。结果:观察组与对照组相比,术中出血率分别为85.00%、51.67%;术后出血率分别为5.00%、23.33%;穿孔率分别为0、1.67%;临床症状严重程度积分分别为2.75±1.28、3.80±1.75;住院天数分别为4.41±0.70天、5.45±2.24天;住院费用分别为4947.83±259.87元、5917.98±1819.19元。结论:冷圈套器切除结肠息肉可提高疗效、降低并发症的发生、减少住院时间及费用,值得临床推广应用。

Objective To observe and evaluate the clinical effect of endoscopic cold snare in resection of colonic polyps.Methods 120 patients with colon polyps(5mm≤diameter of polyp≤10mm)confirmed by colonoscopy and histopathology in the Fourth Hospital of Changsha from January 2017 to November 2018 were selected.They were randomly and equally divided into observation group(cold snare resection)and control group(hot snare resection).The intraoperative bleeding rate,the postoperative bleeding rate,the perforation rate,the score of severity of clinical symptoms,the days and expenses of hospitalization were compared between the two groups treated with polyps.Results Compared between the observation group and control one,the intraoperative bleeding rates were 85.00%and 51.67%,respectively.The postoperative bleeding rates were 5.00%and 23.33%,respectively.The perforation rates were 0 and 1.67%,respectively.The score of severity of clinical symptoms were 2.75±1.28 and 3.80±1.75,respectively.The days of hospitalization were 4.41±0.70 days and 5.45±2.24 days,respectively.The expenses of hospitalization were 4947.83±259.87 RMB and 5917.98±1819.19 RMB,respectively.Conclusion The cold snare resection in the treatment of colonic polyps can improve the curative effect,reduce the incidence of complications and the days and expenses of hospitalization,which is worthy of clinical popularization and application.
