功能性鼻内镜手术治疗鼻-鼻窦炎的疗效及对生活质量的影响

Effect of functional endoscopic sinus surgery on rhinosinusitis and its effect on quality of life

目的:观察功能性鼻内镜手术对鼻-鼻窦炎的治疗效果,及对患者鼻黏液纤毛清除功能、生活质量和炎症反应水平的影响。方法:选取2017年2月~2018年2月在我院接受治疗的鼻-鼻窦炎患者为研究对象,所有患者均接受功能性鼻内镜手术。观察患者治疗前后鼻黏液纤毛清除功能、症状评分、生活质量和炎症反应水平的差异。结果:术后6个月,鼻-鼻窦炎患者的糖精清除时间较术前降低,黏液纤毛清除率和黏液纤毛清除速度较术前增高;术后6个月,鼻-鼻窦炎患者的IL-10、IFN-γ、IL-6和hs-CRP水平均较术前降低;术后6个月,鼻-鼻窦炎患者的头痛、面部疼痛和鼻塞等症状评分低于术前;鼻-鼻窦炎患者的生理功能、生理角色和总体健康等维度得分均高于术前。结论:功能性鼻内镜手术对鼻-鼻窦炎具有较好的治疗效果,可明显改善鼻-鼻窦炎患者的鼻黏液纤毛清除能力和生活质量,具有良好的临床应用价值。

Objective To observe the effect of functional endoscopic sinus surgery(FESS)on nasal mucociliary clearance,quality of life and inflammatory response in patients with sinusitis.Methods Patients with rhinosinusitis treated from February 2017 to February 2018 received functional endoscopic sinus surgery.The nasal mucociliary clearance function,symptom score,quality of life and inflammatory reaction were observed before and after treatment.Results Six months after operation,the glycogen clearance time of patients with rhinosinusitis was shorter than that before operation,the mucociliary clearance rate and mucociliary clearance rate were higher than those before operation.At 6 months postoperatively,the levels of IL-10,IFN-gamma,IL-6 and hs-CRP in patients with rhinosinusitis were lower than those before surgery.Six months after surgery,the scores of headache,facial pain and nasal obstruction in patients with rhinosinusitis were lower than those before surgery;the scores of physiological function,physiological role and general health in patients with rhinosinusitis were higher than those before surgery.Conclusion Functional endoscopic sinus surgery has a good therapeutic effect on rhinosinusitis,can significantly improve the nasal mucociliary clearance ability and quality of life in patients with rhinosinusitis,and has a good clinical value.
