非小细胞肺癌多发脑转移全脑放疗联合局部立体定向放疗的疗效

Efficacy of whole brain radiotherapy combined with local stereotactic radiotherapy for multiple brain metastases from non-small cell lung cancer

目的:比较非小细胞肺癌多发脑转移患者全脑放疗联合或不联合局部立体定向放疗的疗效。方法:收集本院肿瘤科2012年4月~2014年4月收治的非小细胞肺癌多发脑转移患者(脑转移瘤数目≥2个)51例,随机为研究组26和对照组25例,研究组为全脑放疗联合局部立体定向放疗,对照组为全脑放疗,比较两组患者的近期有效率、不良反应、生存时间。结果:研究组近期有效率为92.3%明显高于对照组的60%,两组不良反应发生率分别为75%和60%,无统计学意义,均为0~2级,研究组中位生存时间为21个月明显高于对照组9个月。结论:非小细胞肺癌多发脑转移患者全脑放疗联合局部立体定向放疗较单用全脑放疗疗效好,安全性无差异,能延长患者生存时间。

Objective To compare the efficacy of whole brain radiotherapy combined with or without local stereotactic radiotherapy in patients with multiple brain metastases from non-small cell lung cancer(NSCLC).Methods From April 2012 to April 2014,51 patients with multiple brain metastases from non-small cell lung cancer(the number of brain metastases≥2)were randomly divided into study group(n=26)and control group(n=25).The study group was whole brain radiotherapy combined with local stereotactic radiotherapy,and the control group was whole brain radiotherapy.The short-term effective rate,adverse reactions and survival time of the two groups were compared.Results The short-term effective rates of the study group was 92.3%,was significantly higher than the control group(60%),and about the incidence of adverse reactions,there was no significant difference between the two groups.The median survival time of the study group was 21 months,was significantly higher than the control group(9 months).Conclusion The curative effect of whole brain radiotherapy combined with local stereotactic radiotherapy in patients with multiple brain metastasis of non-small cell lung cancer is better than that of whole brain radiotherapy alone,there is no difference in safety,and can prolong the survival time of patients.
