平均血小板体积和中性粒细胞计数在眩晕患者中鉴别急性脑梗死的价值

The value of mean platelet volume and neutrophil count in the differential diagnosis of acute cerebral infarction in patients with vertigo

目的分析平均血小板体积(MPV)和中性粒细胞计数在眩晕患者中预测急性脑梗死的价值。方法选取郑州人民医院105位以头晕和(或)眩晕为主诉症状的患者,根据急性脑梗死诊断标准分为急性脑梗死组和非急性脑梗死组,比较两组患者一般资料及血液学指标,并对相关因素进行多因素Logistic回归分析。绘制受试者工作特征(ROC)曲线,评价MPV和中性粒细胞计数在眩晕患者中预测急性脑梗死的价值。结果急性脑梗死组和非急性脑梗死组MPV分别为(11.39±1.16)f L和(10.77±0.94)f L,中性粒细胞计数分别为(4.86±1.65)×10^(9)/L和(3.75±1.20)×10^(9)/L,急性脑梗死组MPV和中性粒细胞计数均高于非急性脑梗死组(P<0.05)。多因素Logistic回归分析显示,MPV和中性粒细胞计数对眩晕患者中急性脑梗死的发生有一定的预测价值。ROC曲线分析显示,MPV预测急性脑梗死的曲线下面积为0.650(95%CI为0.533~0.767),最佳截断点是11.450,敏感度为57.9%,特异度为76.1%;中性粒细胞计数预测急性脑梗死的曲线下面积为0.702(95%CI为0.592~0.811),最佳截断点为4.185,敏感度为71.1%,特异度为67.2%。结论MPV和中性粒细胞计数对眩晕患者中急性脑梗死的发生有一定的预测价值。

Objective To analyze the value of mean platelet volume(MPV)and neutrophil count in predicting acute cerebral infarction in patients with vertigo.Methods 105 patients with dizziness and(or)vertigo were selected and divided into acute cerebral infarction group and non-acute cerebral infarction group according to the diagnostic criteria of acute cerebral infarction.The general data and hematological indexes in the two groups were compared,and the related factors were analyzed with logistic regression.Receiver operating characteristic(ROC)curve was drawn to evaluate the value of MPV and neutrophil count in predicting acute cerebral infarction in patients with vertigo.Results The levels of MPV[(11.39±1.16)fL]and neutrophil count[(4.86±1.65)×10^(9)/L]in acute cerebral infarction group were higher than those in non-acute cerebral infarction group[(10.77±0.94)f L,(3.75±1.20)×10^(9)/L],and the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that MPV and neutrophil count had predictive value for the occurrence of acute cerebral infarction in patients with vertigo.ROC curve analysis showed that the area under curve of acute cerebral infarction predicted by MPV was 0.650(95%CI:0.533-0.767)with the best cut-off point was 11.450;the sensitivity was 57.9%,and the specificity was 76.1%.The area under the curve of acute cerebral infarction predicted with the neutrophil count was 0.702(95%CI:0.592-0.811)with the best cut-off point of 4.185,the sensitivity of 71.1%,and the specificity of 67.2%.Conclusion MPV and neutrophil count have predictive value for the occurrence of acute cerebral infarction in patients with vertigo.
