2010-2019年河南省开封市医疗卫生机构传染病网络直报情况

Situation of direct online reports of infectious diseases in medical and health institutions in Kaifeng of Henan,2010-2019

目的分析2010-2019年开封市不同类别卫生机构网络直报情况,及时发现问题,提高传染病报告质量,为进一步制定传染病防控对策提供依据。方法收集2010-2019年开封市医疗卫生机构通过《中国疾病预防控制信息系统》报告的传染病卡个案信息,进行描述性流行病学分析。结果2010-2019年开封市进行网络直报的医疗卫生机构类型共18种,从2010年的127家增加至2019年的146家。其中社区卫生服务中心数量增加明显,从3家增加至16家;乡卫生院数量最多,年平均构成比为54.45%(759/1394)。十年间,医疗卫生机构每年通过网络直报系统报告的传染病个案卡从23374例增加到32650例,年均报告卡26929.40例。综合医院、儿童医院、乡卫生院年均报告卡构成比之和为78.90%(212486/269294),为传染病报告主要类型,其中综合医院年均报告卡构成比最高为40.96%(110307/269294),所报告的病种主要有乙肝、丙肝、其它感染性腹泻病、手足口病、水痘、流行性腮腺炎。结论十年间,开封市传染病网络直报医疗卫生机构数和报告病例数均呈增长趋势,但是医疗卫生机构,特别是综合医院和乡镇级医疗机构报告传染病的信息准确性和敏感性不高,应加强对医务人员的传染病诊断和报告标准的培训,落实首诊负责制,加强考核,落实奖惩。

Objective To analyze the situation of direct online reports of infectious diseases in the medical and health institutions(MHIs)in Kaifeng from 2010 to 2019 for finding the problem in time and providing evidence for the prevention.Methods Data were collected through the China Information System for Disease Control and Prevention from 2010 to 2019.Descriptive epidemiological methods were used to analyze the data of the infectious disease cases reported.Results There were 18 types of direct online reporting MHIs in Kaifeng City from 2010 to 2019.The number of direct online reporting MHIs increased from 127 to 146.The number of the community health service centers increased significantly from 3 to 16.The number of the township Health Centers was the largest accounting for 54.45%(759/1394)of the total on average.During the 10 years,the number of the reported infectious disease cards increased from 23374 to 32650.The average number of the reported infectious disease cards was 26929.40 per year.The general hospitals,children’s hospitals and township health centers were the main network direct reporting institutions,and the reporting cards from these three MHIs and from the general hospitals alone accounted for 78.90%and 40.96%(110307/269294)of the total on average,respectively.The general hospitals had the most reporting cards of all the types’MHIs.Other diseases,hepatitis B,hepatitis C,other infectious diarrhea,hand-foot-and-mouth disease(HFMD),chickenpox,and mumps were the main diseases reported by these three types of MHIs.Conclusion The number of direct online reporting MHIs and the reported cases had shown increasing trend in Kaifeng during the past ten years.However,the accuracy and sensitivity of infectious disease reporting in MHIs was not high,especially in general hospitals and township-level medical institutions.In the future,infectious disease diagnosis and report training for medical staff should be strengthened,the responsibility system for first diagnosis should be implemented completely,the assessment,rewards and punishments related to the report of infectious diseases should be strengthened.
