2017-2019年河南省安阳市11所医院院内感染病例调查

Investigation of nosocomial infection cases in 11 hospitals of Anyang,Henan, from 2017 to 2019

目的分析安阳市11所医院的住院患者院内感染情况,了解我市医院的院内感染特点,为开展院内感染防控和消毒监测工作提供依据和方向。方法采用回顾性分析方法,调查2017-2019年我市11所医院783667例出院患者的医院内感染情况。结果2017-2019年安阳市11所医院,所有出院患者发生院内感染率依次为1.02%、1.14%和1.06%。院内感染部位主要集中在呼吸系统(58.15%)、手术伤口(11.27%)、泌尿系统(10.81%)。综合医院患者院内感染部位主要分布在下呼吸道(48.91%)、泌尿道(15.00%)、手术伤口(10.39%);专科医院患者院内感染部位主要分布在下呼吸道(37.76%)、上呼吸道(24.48%)、手术伤口(13.28%)。夏冬季是发生院内感染的高峰期。上呼吸道冬季感染率明显高于其他季节。结论我市11所医院的院内感染率符合国家《医院感染管理规范(试行)》要求,但医务人员要进一步重视医院感染管理。需继续加强对医院的消毒监测工作,严格落实消毒隔离制度及无菌操作规范,使医院感染率保持在较低水平。

Objective To the analysis of nosocomial infection in 11 hospitals in our city,understand the characteristics of nosocomial infection in hospitals and provide basis and direction for the prevention and control of nosocomial infection and disinfection monitoring.Methods Retrospective analysis was used to investigate the nosocomial infection of 783667 discharged patients from 11 hospitals from 2017-2019.Results The nosocomial infection rate in all patients discharged from the 11 hospitals in Anyang City was 1.02%in 2017,1.14%in 2018,and 1.06%in 2019.Nosocomial infection was mainly concentrated in the respiratory system(58.15%),surgical wound(11.27%),and urinary system(10.81%).In general hospitals,the top three infection sites were located in the lower respiratory tract(48.91%),urinary tract(15.00%),and surgical wounds(10.39%);In special hospitals,the top three infection sites were located in the lower respiratory tract(37.76%),upper respiratory tract(24.48%),and surgical wounds(13.28%).The peak of nosocomial infection was in summer and winter.The rate of infection in the upper respiratory tract in winter was significantly higher than that in other seasons.Conclusion The nosocomial infection rate in the 11 hospitals in our city conforms to the requirements of the Hospital Infection Management Standard(Trial)in our country.However,medical staff should pay more attention to hospital infection management.We need to continue to strengthen the monitoring of hospital disinfection,and strictly implement the disinfection and isolation system as well as aseptic operation specifications to keep the hospital infection rate low.
