天津市中青年艾滋病患者抗病毒治疗依从性及影响因素分析

An analysis of antiretroviral therapy compliance and its influencing factors among young and middle-aged patients of AIDS in Tianjin

目的探讨天津市中青年艾滋病患者接受高效联合抗逆转录病毒治疗(简称:HAART)的依从性及相关影响因素。方法选择2018年10-12月天津市河东区疾控中心管理的符合纳入排除标准的全部中青年艾滋病患者共277例,通过信息系统并采用问卷调查方式收集相关信息,了解抗病毒治疗依从性情况,采用Logistic回归模型对相关因素进行分析,筛选其影响因素。结果277名调查对象中,抗病毒治疗依从性良好(按时按计量服药比例≥95%)252名(90.97%)。Logistic回归结果显示大专及以上文化程度(OR=0.714,95%CI:0.621~0.813)、CD4+T淋细胞临床计数>200个/mm2(OR=0.813,95%CI:0.728~0.902)为保护因素;三种及以上联合用药(OR=1.119,95%CI:1.010~1.213)、用药后不良反应(OR=1.171,95%CI:1.045~1.302)为危险因素。结论天津市中青年艾滋病患者服药依从性较好,可以通过进一步加强抗病毒治疗规范性,提高患者认知,降低不良反应,连续规范指导等提高中青年艾滋病患者依从性,从而保证治疗效果,提高远期生活质量。

Objective To study the compliance and its related influencing factors among young and middle-aged patients of AIDS receiving antiretroviral therapy in Tianjin.Methods 277 young and middle-aged cases of HIV/AIDS managed by Tianjin Hedong CDC from October to December 2018 were selected.The information of antiviral treatment compliance was collected through the information management system and questionnaire survey.Logistic regression model was used to screen its influencing fac-tors.Results Among the 277 participants,252(90.97%)had good antiretroviral treatment compliance(the proportion of actual doses taken on time of those prescribed was greater than 95%).Logistic regression results showed that education level(OR=0.714,95%CI:0.621~0.813),CD4+clinical count>200/mm2(OR=0.813,95%CI:0.728~0.902)were protective factors;and≥3 medications combination(OR=1.119,95%CI:1.010~1.213)and adverse reaction after medication(OR=1.171,95%CI:1.045~1.302)were risk factors associated with the compliance.Conclusion The compliance of middle and young patients of AIDS in Tianjin is good,and it can be improved by further strengthening the standardization of antiretroviral therapy,increasing the related knowledge of AIDS patients,reducing adverse reactions,and continuously conducting standard guidance,so as to ensure the treatment effect and improve the long-term quality of life.
