Analysis of diphtheria antibody levels in healthy population in Henan Province in 2018

摘要 目的了解河南省健康人群白喉抗体水平,为科学制定疫苗接种策略提供依据。方法2018年采用系统抽样的方法选择监测点,采集健康人群血清,用酶联免疫吸附试验(Enzyme-Linked Immunosorbent Assay,ELISA)检测血清中抗白喉类毒素(diphtheria toxoid,DT)抗体水平。结果共检测547份健康人群血清标本,抗白喉类毒素IgG抗体的几何平均浓度(Geometric Mean Concentration,GMC)为0.088 IU/mL,阳性标本(≥0.01 IU/mL)493例,阳性率为90.13%,达到完全保护水平(≥0.1 IU/mL)298例,完全保护率为54.48%。不同性别白喉抗体水平的差异没有统计学意义(P>0.05)。3~4岁组抗体阳性率最高(97.78%),15~19岁组抗体阳性率最低(75.95%)。接种剂次≥4次人群阳性率最高(94.28%),不同接种剂次人群白喉抗体水平、阳性率的差异均有统计学意义(P<0.05)。结论河南省健康人群白喉抗体阳性率较高,但达到完全保护水平的比例较低,需加强人群抗体水平的动态监测,以及时发现免疫薄弱人群。 Objective To get known of diphtheria antibody levels of healthy people in Henan Province,and to provide evidence for scientifically formulating vaccination strategies.Methods In 2018,a systematic sampling method was used to select monitoring points,sera from healthy people were collected,and anti-diphtheria toxoid(DT)antibody levels were detected by Enzyme-Linked Immunosorbent Assay(ELISA).Results A total of 547 serum samples from healthy people were tested.The Geometric Mean Concentration(GMC)of DT IgG antibody levels was 0.088 IU/mL.493 cases were positive(≥0.01 IU/mL),298 cases reached the level of complete protection(≥0.1 IU/mL),and the rate was 54.48%.The difference in diphtheria antibody levels between different genders was not statistically significant(P>0.05).The highest antibody positive rate was observed in the 3~4 year-old group(97.78%),and the lowest was observed in the 15~19 year-old group(75.95%).The highest positive rate was found in populations with inoculation times≥4 times(94.28%).The difference in diphtheria antibody levels and positive rates among populations with different inoculation times were both statistically significant(P<0.05).Conclusions The positive rate of diphtheria antibody levels in healthy population in Henan Province was high,but the percentage of population reaching the full protection level was low.It was necessary to strengthen the dynamic monitoring of antibody levels of healthy population,in order to find weak immunization population in time.
