ICU失禁性皮炎信息化管理模块的构建及应用研究

Design and application of an information module for incontinence-associated dermatitis in ICU

目的构建ICU失禁性皮炎信息化管理模块并验证其临床应用效果。方法以护理程序为理论框架构建失禁性皮炎信息化管理模块,开发了部位评估、分级评估、护理指导、文书记录以及数据监控等模块功能。比较信息化管理模块应用前(2019年5月—7月)与应用后(2019年9月—11月)ICU住院患者的失禁性皮炎发生率及严重程度、护理文书记录准确率,并调查临床护士对该模块的接受度。结果ICU失禁性皮炎信息化管理模块运用后,失禁性皮炎发生率从20.3%下降到10.8%,差异具有统计学意义(χ2=7.903,P=0.005);护理文书记录准确率从52.1%提高至88.0%,差异具有统计学意义(χ2=9.248,P=0.002);护士对该模块的接受度处于较高水平。结论ICU失禁性皮炎信息化管理模块的应用可以降低ICU患者失禁性皮炎发生率,提高护理文书记录质量,同时临床护士对信息化管理模块接受度高。

Objective To construct an information module of ICU incontinence-associated dermatitis(IAD)and verify its effects.Methods Using nursing procedure as the theory framework,we constructed the information module of ICU IAD which included location assessment,grading assessment,nursing guidance,document record,data monitor ing function.Compared the incidence and severity of IAD,the accuracy of the nursing records before(from May to July 2019)and after(from September to November 2019)the application of the module.We investigated nurses’acceptance of the IAD information module.Results After the application,the incidence of IAD was decreased from 20.3%to 10.8%,the difference was statistically significant(χ2=7.903,P=0.005).The accuracy of the nursing records of IAD increased from 52.1%to 88.0%,the difference was statistically significant(χ2=9.248,P=0.002).The results showed that nurses have high acceptance with the overall module.Conclusion The information module of ICU IAD can reduce the rate of incidence and severity of IAD,improve the quality of clinical nursing documents.The nurses have high acceptance with the module.
