MRA联合ABCD2评分在TIA患者中的临床应用

目的探究磁共振血管成像(MRA)联合早期卒中风险预测工具(ABCD2评分)在短暂性脑缺血发作(TIA)患者中的应用价值。方法将2022年05月-2023年05月被临床确诊为TIA并住院的患者60例作为研究对象,所有患者均接受MRA、ABCD2评分检查,分析不同检查方式对TIA患者的检出情况及其对患者预后的影响。结果从MRA检查结果来看,正常或轻度狭窄组、中度狭窄组、严重狭窄或闭塞组检出率分别为43.33%、36.67%和20.00%,其中,无论是一周还是一个月,严重狭窄或闭塞组、中度狭窄组TIA患者脑梗死的发生率均比较高;从ABCD2评分检查结果来看,低危组、中危组和高危组检出率分别为60.00%、36.67%和3.33%,无论是一周还是一个月,中危组、高危组TIA患者脑梗死的发生率均比较高;联合应用下发现狭窄≥50%且≥4分的TIA患者脑梗死的发生率最高,其次为狭窄<50%且≥4分,最低的为狭窄<50%且<4分的TIA患者(P<0.05)。结论ABCD2评分联合MRA检查在TIA患者中的检出效果较优,且基于此结果可有效了解到TIA患者发生脑梗死的可能性。

Objective To explore the application value of magnetic resonance angiography(MRA)combined with early stroke risk prediction tool(ABCD2 score)in patients with transient ischemic attack(TIA).Methods 60 patients clinically diagnosed and hospitalized as TIA in Hongqi Hospital Affiliated to Mudanjiang Medical College from 2022.05 to 2023.05 were selected as the objects of this study.All patients received MRA and ABCD2 score examination,and the influence of different examination methods on the detection of TIA patients and their prognosis was analyzed.Results From the MRA examination results,the detection rates of normal or mild stenosis group,moderate stenosis group,severe stenosis or occlusion group were 43.33%,36.67%and 20.00%,respectively.Among them,the incidence of cerebral infarction in TIA patients in severe stenosis or occlusion group and moderate stenosis group was higher no matter one week or one month.According to the results of ABCD2 scores,the detection rates of low-risk group,medium-risk group and high-risk group were 60.00%,36.67%and 3.33%,respectively.The incidence of cerebral infarction of TIA patients in medium-risk group and high-risk group was relatively high no matter one week or one month.Under combined application,TIA patients with stenosis≥50%and≥4 score had the highest incidence of cerebral infarction,followed by TIA patients with stenosis<50%and≥4 score,and TIA patients with stenosis<50%and<4 score were the lowest(P<0.05).Conclusions ABCD2 score combined with MRA examination has better detection effect in TIA patients,and based on this result,the possibility of cerebral infarction in TIA patients can be efectively understood.
