带状疱疹与大气PM_(10)关系的初步研究 被引量:1

Primary study on the association between herpes zoster and PM_(10)

摘要 目的探索大气PM_(10)与带状疱疹的关系。方法选取2015—2020年间天津一所皮肤病专科医院11457例住院患者,以是否诊断为带状疱疹划分为病例组(n=4213)及对照组(n=7244),收集患者年龄、性别、家庭地址、入院节气等信息,使用时空模型卫星反演技术进行PM_(10)个体暴露评估,以入院日期前5年日平均暴露浓度代表个体暴露水平。使用非条件logistic回归评估PM_(10)暴露与带状疱疹的关系。结果11457例住院患者中,带状疱疹4213例,非带状疱疹7244例;年龄分别为(63.5±13.0)、(57.2±16.9)岁,PM_(10)个体暴露水平分别为(107.2±3.0)、(106.8±6.5)μg/m^(3)。logistic回归显示,大气PM_(10)每上升10μg/m^(3),带状疱疹患者住院风险上升14.4%[OR(95%CI):1.144(1.029~1.271)]。并未发现PM_(10)致带状疱疹与性别、年龄有交互作用。结论本研究为带状疱疹危险因素的研究提供了可能的线索,应加强大气污染治理及个体防护。 Objective To investigate the association between long-term exposure to inhalable particulate matter(PM_(10))and herpes zoster in Tianjin.Methods All patients were recruited from hospital admissions in a tertiary dermatology hospital in Tianjin from 2015 to 2020.The patients were divided into case group(n=4213)and control group(n=7244)according to diagnosis of herpes zoster.The sociodemographic information of the subjects,such as age,gender,home address,solar term of hospital admissions were collected.A satellite-based spatiotemporal model was used to estimate PM_(10)concentration in the individual level.The average daily concentration in five years prior to the diagnosis was used to represent the individual exposure levels.Unconditional logistic regression was used to estimate the association between PM_(10)exposure and hospital admissions of herpes zoster.Results A total of 11457 participants were selected in this study,including 4213 cases in the case group and 7244 cases in the control group;The ages were(63.5±13.0)and(57.2±16.9)years old,and the mean personal exposure level to PM_(10)were(107.2±3.0)and(106.8±6.5)μg/m^(3)respectively.Logistic regression showed that the increase of14.4%[OR(95%CI):1.144(1.029-1.271)]hospital admissions of herpes zoster was associated with each increase of 10μg/m^(3)PM_(10).No significant interaction effect was observed between PM_(10),gender and age.Conclusion PM_(10)can act as a risk factor for herpes zoster,and environmental pollution control and individual protection should be strengthened to promote public health.
