电切镜下钬激光碎石术治疗膀胱结石的疗效观察及安全性评估

Observation of Curative Effect and Safety Evaluation of Holmium LaserLithotripsy Under Electroscope for Bladder Calculi

目的探讨电切镜下钬激光碎石术治疗膀胱结石的疗效观察及安全性。方法选择103例膀胱结石患者,信封法将患者随机分为两组,52例患者采用电切镜下钬激光碎石治疗(HL组),51例患者采用气压弹道碎石治疗(PL组)。对比两组患者疗效、手术时间、住院时间、结石排净率、并发症的差异。结果HL组患者治疗总有效率为94.23%,PL组80.39%,两组差异显著(P<0.05)。HL组患者手术时间、住院时间短于PL组[(32.64±9.31)min vs.(48.51±15.20)min、(3.03±0.71)d vs.(4.95±1.04)d,P<0.05],结石排净率高于PL组(96.15%vs.80.39%,P<0.05)。HL组并发症发生率低于PL组(0.00%vs.13.73%,P<0.05)。结论电切镜下钬激光在治疗膀胱结石的临床疗效和安全性方面优于气压弹道碎石治疗。

Objective To investigate the efficacy and safety of holmium laser lithotripsy under electroscope in the treatment of bladder calculi.Methods 103 patients with bladder calculi were selected.The patients were randomly divided into two groups by envelope method,that is,the Group HL:52 patients who accepted electroscope with holmium laser lithotripsy,and the Group PL:51 patients who were treated with electroscope with pneumatic ballistic lithotripsy.The difference of curative effect,operation time,hospitalization time,stone clearance rate and complications between the two groups were compared.Results The total effective rate was 94.23%in group HL and 80.39%in group PL.There was a significant difference between the two groups(P<0.05).The operation time and hospitalization time of group HLwere shorter than those of group PL[(32.64±9.31)min VS(48.51±15.20)min、(2.03±0.71)d VS(3.95±1.04)d,P<0.05].The stone clearance rate of group HL was higher than that of group PL(96.15%VS 80.39%,P<0.05),and the complication rate of group HL was lower than that of group PL(0.00%VS 13.73%,2=10.055,P<0.05).Conclusion Holmium laser lithotripsy under electroscope is superior to pneumatic lithotripsy in the treatment of bladder calculi in clinical efficacy and safety.
