腓骨头上入路治疗胫骨后外侧平台骨折与传统前外侧手术入路比较

Comparison of Suprafibular Head Approach and Traditional AnterolateralApproach in the Treatment of Posterolateral Tibial Plateau Fractures

目的腓骨头上入路治疗胫骨后外侧平台骨折与传统前外侧手术入路效果及预后的比较。方法对胫骨骨折患者100例进行回顾性分析。按照患者治疗方式的不同,分为研究组和对照组。研究组应用经皮微创锁定加压钢板进行固定治疗,对照组应用闭合复位带锁髓内钉进行固定治疗。术后,评估两组手术优良率及并发症情况。结果研究组的术后优良率明显高于对照组,差异有统计学意义(P<0.05)。研究组患者术中出血量明显少于对照组,差异有统计学意义(P<0.05)。研究组的手术时间、住院时间及骨折愈合时间均明显短于对照组,差异有统计学意义(P<0.05)。术后,研究组的并发症发生率(8.00%)明显低于对照组(28.00%),两组比较差异有统计学意义(χ2=6.7751,P=0.0092)。结论腓骨头上入路治疗胫骨后外侧平台骨折效果明显优于传统前外侧手术入路的效果,疗效显著,值得广泛应用。

Objective To study the application effect and prognosis of two kinds of surgical methods on tibial fracture.Methods 100 patients with tibial fracture were retrospectively analyzed.According to the different treatment methods of patients,they were divided into study group and control group.The study group was treated with minimally invasive percutaneous locking compression plate,while the control group was treated with closed reduction and locking intramedullary nail.After operation,the excellent and good rate and complications of the two groups were evaluated.Results The excellent and good rate of the study group was significantly higher than that of the control group(P<0.05).The intraoperative blood loss of the study group was significantly less than that of the control group(P<0.05).The operation time,hospitalization time and fracture healing time of the study group were significantly shorter than those of the control group(P<0.05).After operation,the incidence of complications in the study group(8.00%)was significantly lower than that in the control group(28.00%),with statistical significance(χ2=6.7751,P=0.0092).Conclusion The effect of suprafibular head approach in the treatment of posterolateral tibial plateau fractures is obviously better than that of the traditional anterolateral approach,which is worthy of wide application.
