62例新型冠状病毒肺炎患者的实验室指标分析

Laboratory test analysis of sixty-two COVID-19 patients

目的:分析新型冠状病毒肺炎(COVID-19)患者的实验室检查特点。方法:以2020年1月31日至2月25日期间武汉大学人民医院东院收治的62例确诊为COVID-19患者作为研究对象,将患者分为普通型、重型和危重型,分析患者的基本信息、入院后第一次实验室检查包括血常规、C反应蛋白、降钙素原、生化检测、细胞免疫、体液免疫等,对比不同疾病严重程度患者之间临床特征的差异。结果:危重症病例与普通患者和重症患者相比:淋巴细胞计数、CD3、CD4、CD8细胞水平均明显降低,白细胞计数、中性粒细胞计数、CRP、PCT水平均明显升高,差异有统计学意义(P<0.05)。结论:危重型新型COVID-19患者入院后第一次实验室检查结果提示免疫细胞减少,炎症反应指标升高,在临床救治COVID-19患者过程中需密切关注相关指标水平,以实现对危重症患者的早期识别。

Objective:To analyze the laboratory characteristics and clinical outcomes of patients infected with COVID-19.Methods:We collected and analyzed the data of sixty-two patients who were admitted to Renmin Hospital of Wuhan University and were laboratory-confirmed as SARS-CoV-2 infection between January 31 and February 25,2020.The cases were divided into regular group,severe group,and critically ill group according to the sixth edition guideline for COVID-19 diagnosis and treatment of China.The laboratory tests included blood routine,C-reactive protein(CRP),procalcitonin(PCT),biochemical detections,cellular immunity and humoral immunity indices,etc.,and the laboratory test results were anlayzed according to the clinical features and severities of disease.Results:As compared with those in regular group and severe group,the indices in critically ill group such as lymphocyte count,and levels of CD3,CD4,and CD8 were significantly lower,while the leukocyte count,neutrophil count,CRP and PCT levels were significantly higher,with significant difference(all P<0.05).Conclusion:The first laboratory test results of critically ill patients infected with SARS-CoV-2 after admission indicated decreased immune cells and increased inflammatory response indicators.During the process of clinical treatment,special attention should be paid to the level of relevant indicators in order to realize the early recognition of critically ill patients.
