选择性动脉栓塞术在脊柱肿瘤外科术前的应用价值

Value of selective arterial embolization before surgical treatment of spinal tumors

目的:探讨脊柱肿瘤外科治疗前经导管动脉栓塞术(TAE)的应用价值。方法:选取2016年1月至2019年3月我院38例脊柱肿瘤患者,分为栓塞组(n=18)和非栓塞组(n=20)。栓塞组在手术前先行椎体肿瘤TAE,再进行外科椎体肿瘤切除及重建术;非栓塞组直接行椎体肿瘤切除及重建术,观察手术中的相关指标并进行对比分析。结果:栓塞组的术中出血量平均约2152.78 mL,手术时间约2~4.5 h;非栓塞组中,术中出血量平均约4353.13mL,手术时间约为5~8 h,两组术中平均出血量与手术时间的差异有统计学意义(P<0.01)。所有患者随访1~3年各项功能恢复满意,椎体的稳定性良好。结论:外科治疗前行椎体肿瘤TAE是安全、有效和可行的治疗手段,可明显降低术中出血量、缩短手术时间,提高手术成功率。

Objective:To explore the value of transcatheter arterial embolization(TAE)before the surgical treatment of spinal tumors.Methods:During the period from January 2016 to March 2019 in our hos-pital,38 patients with spinal tumor were selected and divided into the embolization group(n=18)and the non-embolization group(n=20).In the embolization group,TAE was performed before surgery,followed by surgical resection and reconstruction of vertebral tumor.In the non-embolization group,vertebral tumor resection and reconstruction were performed directly.Results:The average intraoper-ative blood loss of the embolization group was 2152.78 mL,and the operative time ranged from 2 to 4.5 hours.In the non-embolization group,the average intraoperative blood loss was 4353.13 mL and the operative time ranged from 5 to 8 hours.The difference between the two groups in the aver-age intraoperative blood loss and the operative time was statistically significant(P<0.01).All pa-tients were followed up for 1-3 years with satisfactory functional recovery and good vertebral body sta-bility.Conclusion:TAE before the surgical treatment of spinal tumors is a safe,effective and feasible treatment method,which can significantly reduce intraoperative blood loss,shorten the operation time and improve the success rate of surgery.
