摘要
目的:探讨早期开髓减压引流术联合氢氧化钙治疗牙髓炎的疗效及对血清Toll样受体4(TLR4)、CX3C趋化因子(CX3CL1)的影响。方法:随机选择2014年1月-2017年1月在南京市口腔医院就诊的牙髓炎患者120例,分为观察组和对照组,各60例。对照组给予开髓减压引流术,观察组予开髓减压引流术+氢氧化钙糊剂治疗,观察两组患者治疗14 d后的疗效、疼痛评分以及血清TLR4、CX3CL1的水平。结果:观察组的总有效率为95. 00%,对照组为75. 00%,差异有意义(P<0. 05);治疗后VAS评分显著下降,且观察组下降更显著(P<0. 05);治疗后两组患者的血清CX3CL1、TLR4水平均显著下降,且观察组下降更显著(P<0. 05)。结论:早期开髓减压引流术联合氢氧化钙治疗牙髓炎的治疗效果较好,可以有效地改善患者症状,降低血清TLR4、CX3CL1的水平,在临床上可广泛使用。
Objective: To investigate the curative effect of early decompression drainage combined with calcium hydroxide treatment for pulpitis and its effect on Toll like receptors 4(TLR4) and CX3C motif ligand 1(CX3CL1).Methods: A total of 120 cases of pulpitis patients were randomly selected from January 2014 to January 2017 in Nanjing Stomatological Hospital, and were divided into observation group and control group, with 60 cases in each group. The control group was treated with decompression and drainage of the pulp, and the observation group was treated with decompression and drainage of the pulp plus Calcium Hydroxide Paste. Fourteen days after treatment, the clinical efficacy, the VAS scores and the level of serum TLR4 and CX3CL1 were observed.Results: The total effective rate of observation group was 95%, which was significantly higher than that of control group with 75%(P<0. 05). After treatment, the VAS scores were significantly decreased in both groups, and the observation group decreased more significantly(P<0. 05). After treatment, the serum CX3CL1 and TLR4 levels of the two groups decreased significantly, and the observation group decreased more significantly(P<0. 05).Conclusion: The effect of early treatment of open decompression drainage combined with calcium hydroxide for pulpitis is better, which can effectively improve symptoms, reduce the serum TLR4 and CX3CL1 levels, and can be widely used in clinic.
