维持性血液透析患者合并新型冠状病毒肺炎2例报告

Coronavirus disease 2019 in patients on maintenance hemodialysis:report of two cases

目的:分析2例维持性血液透析患者合并新型冠状病毒肺炎患者的临床特征。方法:分析武汉大学中南医院肾病内科收治2例尿毒症合并新型冠状病毒肺炎的临床资料,并结合文献,对此疾病的特征及诊治情况进行分析。结果:2例肺炎患者均伴有呼吸道症状;血液常规表现为白细胞总数正常或减低,淋巴细胞计数减少,降钙素原、IL-6升高;胸部CT表现为单发或多发的斑片状磨玻璃影,伴有小叶间隔增厚。结论:维持性血液透析患者合并新型冠状病毒肺炎的临床特征与一般病毒性肺炎类似;高分辨率CT有助于鉴别诊断,隔离区连续性肾脏替代治疗有助于防止疾病扩散。

Objective: To analyze the clinical characteristics of coronavirus disease 2019(COVID-19) in patients on maintenance hemodialysis.Methods: The clinical data of two COVID-19 cases on maintenance hemodialysis in Zhongna Hospital of Wuhan University in January 2020 were analyzed, the related literatures were reviewed.Results: The main symptoms of COVID-19 pneumonia were fever and other respiratory or systemic symptoms. The blood test of the patients showed normal or decreased white blood cell count, and decreased lymphocyte count. Serum PCT and IL-6 levels were significantly increased, while albumin levels were decreased. The CT features were ground glass like shadow and consolidation.Conclusion: The clinical characteristics of COVID-19 are similar to those of common viral pneumonia. High resolution CT is of great value in the differential diagnosis of this disease. Continuous renal replacement therapy in the isolation ward can prevent the spread of COVID-19.
