eCASH理念的进阶式肺康复护理对重症肺炎机械通气患者睡眠质量及肺功能的影响

Effect of Advanced Pulmonary Rehabilitation Nursing Based on eCASH Concept on Sleep Quality and Lung Function in Patients with Severe Pneumonia Induced by Mechanical Ventilation

目的探讨基于早舒及浅镇静(eCASH)理念进阶式肺康复护理对重症肺炎机械通气患者睡眠质量及肺功能影响。方法选取2018年5月—2023年5月该院收治的60例重症肺炎机械通气患者为研究对象,按随机数字表法将其分成观察组与对照组,各30例。对照组采用常规护理干预,观察组采用eCASH理念进阶式肺康复护理干预。比较两组干预前后匹兹堡睡眠质量指数(PSQI)评分,干预前后第1秒用力呼气容积占预计值百分比(FEV_(1)%)、用力肺活量(FVC)、最大呼气流速(PEFR)、患者满意度情况。结果干预1、3个月,观察组PSQI评分均低于对照组,组间差异有统计学意义(P<0.05);干预3个月,观察组FEV_(1)%、FVC、PEFR指标水平均大于对照组,组间差异有统计学意义(P<0.05)。观察组的护理总满意率为96.67%,高于对照组的76.67%,差异有统计学意义(P<0.05)。结论针对重症肺炎行机械通气治疗的患者,予以eCASH理念的进阶式肺康复护理能显著提高患者的睡眠质量、肺功能,同时可提高患者满意度,值得推广。

Objective To explore the effect of advanced pulmonary rehabilitation nursing based on the concept of early relief and light sedation(eCASH)on sleep quality and lung function of patients with severe pneumonia undergoing mechanical ventilation.Methods Selecting 60 mechanically ventilated patients with severe pneumonia admitted to the hospital from May 2018 to May 2023 as the research subjects,and were divided into the observation group and the control group according to the random number table method,each with 30 cases.Patients in the control group received routine nursing intervention,while patients in the observation group received eCASH concept advanced pulmonary rehabilitation nursing intervention.The Pittsburgh Sleep Quality Index(PSQI)scores,forced expiratory volume in one second(FEV_(1)%),forced vital capacity(FVC),maximum expiratory flow rate(PEFR)and patient satisfaction before and after intervention were compared between the two groups.Results One and three months after the intervention,the PSQI scores of the observation group were lower than those of the control group,and the differences between groups werestatistically significant(P<0.05).After 3 months of intervention,the levels of FEV_(1)%,FVC and PEFR in the observation group were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).The total nursing satisfaction rate of the observation group was 96.67%,which was higher than 76.67%of the control group,and the difference was statistically significant(P<0.05).Conclusion For severe pneumonia patients with mechanical ventilation,the application of eCASH concept of advanced lung rehabilitation nursing can significantly improve the sleep quality,lung function,and satisfaction of patients,which is worthy of promotion.
