麦默通旋切术与乳晕弧形切口治疗乳腺纤维瘤的临床疗效观察

Clinical observation of maimerton circumcision and areola arc incision in the treatment of breast fibroma

目的比较麦默通旋切术与乳晕弧形切口治疗乳腺纤维瘤的临床疗效。方法回顾性分析160例女性乳腺纤维瘤患者的临床资料。麦默通组86例,采用超声定位下麦默通旋切术,乳晕组74例,采用环乳晕弧形切口手术方式。比较两组手术时间、切口长度、术中出血量、术后甲级愈合率、术后美容效果满意率、术后并发症及复发率、住院费用和住院天数等。结果麦默通组平均手术时间、平均切口长度、术中出血量明显少于环乳晕组(P<0.05);麦默通组患者平均住院费用较环乳晕组明显增加(P<0.05);麦默通组术后甲级愈合率、术后美容效果、术后并发症及短期复发率、住院天数与环乳晕切口组相比较差异无统计学意义(P>0.05)。结论采用麦默通和环乳晕弧形切口治疗乳腺纤维瘤的临床疗效确切,均能够达到美容效果,但是麦默通手术具有手术时间短、手术切口小、术中出血少的优点,但增加了患者的费用。

Objective Compared the clinical effect of mammotome rotatory cutting with surroundmammary areola incision in the surgery of mammary fibroadenoma..Methods The clinical data of 160 mammary fibroadenoma patients with surgery of mammotome rotatory cutting(86),and surround-mammary areola incision(74)were retrospectively analyzed.The time of operation,the length of incision,mean bleeding volum,postoperative class-A healing rate and patient satisfaction of cosmetic effect,postoperative complications and recurrence rate,days of hospital stay and hospital fees were compared of the two groups.Results The time of operation,the length of incision,mean bleeding volum of the mammotome rotatory cutting group were significantly less than the surroundmammary areola incision group(P<0.05).While the hospital fees were significantly much than than the surroundmammary areola incision group(P<0.05).Then the postoperative class-A healing rate and patient satisfaction of cosmetic effect,postoperative complications and recurrence rate and days of hospital stay were had no differentiation of the two groups(P>0.05).Conclusion Mammotome rotatory cutting and surround-mammary areola incision in the surgery of mammary fibroadenoma all has the function of cosmetic effects.The mammotome rotatory cutting has much more advantage in less opretion time,less length of incision less and blooding volum.But it is improved the burden of patients.
