骨质疏松性胸腰椎骨折应用高黏度骨水泥PVP和PKP治疗的对比分析

Comparative analysis of high viscosity bone cement PVP and PKP in the treatment of osteoporotic thoracolumbar fractures

目的骨质疏松性胸腰椎骨折应用高黏度骨水泥PVP和PKP治疗的对比分析。方法选取我院2019年7月-2020年2月中的78例骨质疏松性胸腰椎骨折患者作为治疗研究对象,分成骨水泥经皮椎体成形术(PVP组)和球囊扩张椎体后凸成形术(PKP组),两组各39例;对比两组手术前、后2h、1个月、5个月VAS评分、Oswestry功能障碍指数问卷表(ODI)值、两组患者术中注入骨水泥量、椎体平均增高高度值、术中出血量、手术时间。结果相比于手术前,两组患者手术后2h、1个月、5个月VAS评分和ODI值比较,P<0.05差异有统计学意义。两组注入骨水泥量比较,P>0.05差异无统计学意义。PKP组椎体平均增高值高于PVP组,P<0.05差异有统计学意义,PVP组术中出血量低于PKP组,手术时间短于PKP组,P<0.05差异有统计学意义。结论骨质疏松性胸腰椎骨折应用高黏度骨水泥PVP和PKP治疗的对比分析中,PKP组治疗方法对于帮助椎体高度恢复有一定的好处,但是PVP组的治疗方法可以有效减少术中出血量。降低手术的时间。

Objective To compare and analyze the treatment of osteoporotic thoracolumbar fracture with high viscosity bone cement PVP and PKP.Methods 78 patients with osteoporotic thoracolumbar fractures in our hospital from July 2019 to February 2020 were divided into cement percutaneous vertebroplasty(PVP)and balloon dilated kyphoplasty(PKP),39 cases in each group.The VAS score,the(ODI)value of Oswestry dysfunction index,the amount of bone cement injected into the operation,the average height of vertebral body,intraoperative blood loss,operation time and treatment effect were compared between the two groups before operation,2 hours,1 month and 5 months after operation.Results compared with those before operation,there were significant differences in VAS score and ODI between the two groups at 2 hours,1 month and 5 months after operation.There was no significant difference in the amount of bone cement injected between the two groups(P>0.05).The average increase of vertebral body in PKP group was higher than that in PVP group,and the difference was statistically significant.The intraoperative blood loss in PVP group was lower than that in PKP group,the operation time was shorter than that in PKP group,and the effective rate was higher in PVP group.Conclusion in the comparative analysis of the treatment of osteoporotic thoracolumbar fracture with high viscosity bone cement PVP and PKP,the treatment method of PKP group is beneficial to the recovery of vertebral body height,but the treatment method of PVP group can effectively reduce the amount of intraoperative bleeding.Reduce the time of operation.
