阴道镜联合液基细胞学技术和Hpv检测在宫颈癌前病变诊断中的价值探讨

The value of colposcopy combined with liquid-based cytology and HPV detection in the diagnosis of cervical precancerous lesions

目的分析阴道镜联合液基细胞学技术和Hpv检测在宫颈癌前病变诊断中的价值。方法本次研究对象是宫颈癌前病变患者650例TCT和(或)HPV阳性,时间在2019年3月至2020年2月,将患者随机分成两组,参照组(n=325)均予以HR-HPV和TCT检测,实验组(n=325)在HR-HPV和TCT检测同时联合阴道镜定位活检术,对比二组患者的宫颈上皮内瘤变阳性率。结果实验组患者的宫颈上皮内瘤变阳性率为50.15%,高于参照组的31.38%,组间数据对比存在显著差异(P<0.05);对实验组确定为宫颈癌的患者进行手术治疗,其中3例宫颈癌术后Hpv持续阳性,术后随访中定期进行阴道镜定位下活检,最后均确诊为阴道癌。结论为HR-HPV和TCT检测结果为阳性者,行阴道镜定位活检具有一定优势,可有效提升宫颈癌前病变的诊断准确率,以便临床为患者采取及时、有效的治疗措施,利于预后。

Objective To analyze the value of colposcopy combined with liquid-based cytology and HPV detection in the diagnosis of cervical precancerous lesions.Methods 650 patients with the object of study is cervical cancer lesion before TCT and HPV positive,time in March 2019 to February 2020,the patients were randomly divided into two groups,control group(n=325)were to HR-HPV and TCT detection,the experimental group(n=325)in the HR HPV and TCT detection at the same time combined colposcope positioning biopsy,compared two groups of patients with cervical intraepithelial neoplasia were positive.Results The positive rate of cervical intraepithelial neoplasia in the experimental group was 50.15%,which was higher than that in the reference group(31.38%),and there was significant difference between the two groups(P<0.05).The patients identified as cervical cancer in the experimental group received surgical treatment,among which 3 patients were continuously positive for HPV after cervical cancer surgery.During the postoperative follow-up,colposcopic biopsy was performed regularly,and finally all patients were diagnosed as vaginal cancer.Conclusion For patients with positive HR-HPV and TCT detection results,colposcopy biopsy has certain advantages,which can effectively improve the diagnostic accuracy of cervical precancerous lesions,so that timely and effective treatment measures can be taken for the patients clinically,which is conducive to the prognosis.
