摘要
目的探讨基于目标策略的针对性护理干预在急性药物性肝损伤(drug induced liver injury,DILI)患者中的应用效果。方法收集2017年1月—2021年12月大连市公共卫生临床中心因急性DILI就诊的患者100例,随机分为对照组和研究组各50例。对照组采用常规护理方案;研究组采用基于目标策略的针对性护理干预方案,比较两组患者干预15 d后的症状缓解情况、营养水平及心理健康状态。结果研究组患者黄疸持续时间、乏力持续时间、肝区疼痛持续时间、丙氨酸转氨酶恢复时间、天门冬氨酸转氨酶恢复时间均短于对照组,差异有统计学意义(P<0.05)。研究组患者总蛋白、白蛋白水平均高于对照组,差异有统计学意义(P<0.05)。研究组抑郁-焦虑-压力量表简体中文版-21各条目及总分均低于对照组,差异有统计学意义(P<0.05)。结论基于目标策略的针对性护理干预用于急性DILI患者的护理,能够进一步促进病情的缓解和肝功能的恢复,并且进一步改善患者的心理状态,值得临床采用。
Objective To explore the effect of targeted nursing intervention based on goal strategy in patients with acute drug-induced liver injury(DILI).Methods A total of 100 patients with acute DILI in our hospital from January 2017 to December 2021 were collected and divided into control group and study group with 50 patients each.The control group was given routine nursing care,and the study group adopted a targeted nursing intervention program based on goal strategy.The symptom relief,nutritional level and mental health status after 15 days of intervention were compared between two groups.Results The duration of jaundice,fatigue,liver pain,recovery time of alanine aminotransferase and aspartate aminotransferase in the study group were significantly shorter than those in the control group(P<0.05).The levels of total protein and albumin in the study group were significantly higher than those in the control group(P<0.05).All items and total scores of DASS-21 in the study group were significantly lower than those in the control group(P<0.05).Conclusion Targeted nursing intervention based on goal strategy can further promote the remission of the disease and the recovery of liver function,and further improve the psychological status of patients with acute DILI,which is worthy of clinical application.
