非小细胞肺癌患者癌组织中BRCA1和p53表达与放疗敏感性的相关性 被引量:2

Correlation of BRCA1 and p53 expression in cancer tissues with radiosensitivity in patients with non-small cell lung cancer

摘要 目的探讨非小细胞肺癌患者癌组织中乳腺癌1号基因(BRCA1)和p53表达与放疗敏感性的相关性。方法选取河南科技大学第一附属医院2019年1月至2022年1月收治的112例非小细胞肺癌患者为研究对象,采用三维适形放射治疗,放疗后按照实体肿瘤疗效评价标准(RECIST)1.1版分为完全缓解、部分缓解、稳定或无变化、进展。将完全缓解、部分缓解纳入放疗有效组,稳定或无变化、进展纳入放疗无效组。比较放疗有效组和放疗无效组患者的临床病理特征和癌组织中BRCA1及p53的阳性表达率,使用Logistic回归分析探究可能影响非小细胞肺癌患者放疗敏感性的危险因素,使用Spearman分析癌组织中BRCA1和p53表达与非小细胞肺癌患者放疗敏感性的相关性。结果放疗后完全缓解46例,部分缓解32例,纳入放疗有效组(n=78);稳定或无变化21例,进展13例,纳入放疗无效组(n=34)。放疗有效组与放疗无效组患者在性别、肿瘤直径、病理分型、淋巴结转移方面比较,差异未见统计学意义(P>0.05),在年龄、病理分期、BRCA1阳性表达率、p53阳性表达率比较差异有统计学意义(P<0.05)。Logistic回归分析显示,年龄>60岁、BRCA1阳性、p53阳性是非小细胞肺癌患者三维适形放疗敏感性的危险因素(P<0.05)。Spearman相关性分析显示,癌组织中BRCA1和p53的阳性表达率与非小细胞肺癌患者放疗敏感性呈负相关(r=-0.476、-0.531,P<0.05)。结论非小细胞肺癌患者癌组织中BRCA1和p53表达与放疗敏感性关系密切,可用于患者放疗疗效的评估。 Objective To investigate the correlation of the expression of breast cancer gene 1(BRCA1)and p53 in cancer tissues with the radiosensitivity of non-small cell lung cancer patients.Methods A total of 112 patients with non-small cell lung cancer admitted to the First Affiliated Hospital of Henan University of Science and Technology from January 2019 to January 2022 were selected as the study subjects.Three-dimensional conformal radiotherapy was used.After radiotherapy,the patients were divided into complete remission,partial remission,stable or no change,and progression according to the evaluation standard of solid tumor efficacy(RECIST)version 1.1.Complete and partial remission were included in the effective group of radiotherapy,and stable or no change and progress were included in the ineffective group of radiotherapy.The clinicopathological characteristics and the positive expression rate of BRCA1 and p53 in cancer tissues of patients in the radiotherapy effective group and the radiotherapy ineffective group were compared.Logistic regression analysis was used to explore the risk factors that might affect the radiosensitivity of patients with non-small cell lung cancer,and Spearman was used to analyze the correlation between the expression of BRCA1 and p53 in cancer tissue and the radiosensitivity of patients with non-small cell lung cancer.Results Forty-six 0 cases of complete remission and 32 cases of partial remission after radiotherapy were included in the radiotherapy effective group(n=78);21 cases were stable or unchanged,13 cases were advanced,and were included in the radiotherapy ineffective group(n=34).There were no significant differences in gender,tumor diameter,pathological classification or lymph node metastasis between the effective group and the ineffective group(P>0.05).There were significant differences in age,pathological stage,BRCA1 positive expression rate and p53 positive expression rate(P<0.05).Logistic regression analysis showed that age>60 years old,BRCA1 positive and p53 positive were risk factors of three-dimensional conformal radiotherapy sensitivity in patients with non-small cell lung cancer(P<0.05).Spearman correlation analysis showed that the positive expression rate of BRCA1 and p53 in cancer tissue was negatively correlated with the radiosensitivity of non-small cell lung cancer patients(r=-0.476,-0.531,P<0.05).Conclusions The expression of BRCA1 and p53 in cancer tissue is closely correlated with the radiotherapy sensitivity of patients with non-small cell lung cancer,which can be used to evaluate the radiotherapy effect of patients.
