摘要
目的研究急性心肌梗死(AMI)患者采用主动脉内球囊反搏(IABP)辅助治疗后,对患者细胞外基质(ECM)蛋白、细胞外基质(ECM)辅助性分子及血清生长因子-15(GDF-15)水平的影响。方法选择包头市中心医院2018年4月至2021年3月收治的急性心肌梗死患者180例,按照随机数字表法分为对照组和观察组,每组90例。对照组行经皮冠状动脉介入术治疗,观察组患者在对照组基础上联合主动脉内球囊反搏辅助治疗。比较治疗前后血清生长分化因子-15(GDF-15)、血心肌梗死三项以及Ⅲ型胶原、Ⅳ型胶原、粘连蛋白IOD值变化,观察两组患者治疗前后心功能以及血流动力学指标的变化。结果治疗后两组患者的血清心肌梗死三项指标与治疗前比较均明显降低(P<0.05),治疗后两组患者的GDF-5水平、Ⅲ型胶原、Ⅳ型胶原、粘连蛋白IOD值、血流动力学指标与治疗前比较明显升高(P<0.05),治疗后两组患者心功能指标LVED水平降低、LVEF水平升高(P<0.05)。且观察组上述指标改善更显著(P<0.05)。结论IABP辅助治疗AMI可以明显改善其心功能和血流动力学指标,升高血清GDF-15水平以及Ⅲ型胶原、Ⅳ型胶原、粘连蛋白IOD值,为PCI血运重建提供条件。
Objective To study the effects of intra aortic balloon counterpulsation(IABP)on extracellular matrix(ECM)protein,extracellular matrix(ECM)helper molecules and serum growth factor-15(GDF-15)levels in patients with acute myocardial infarction.Methods A total of 180patients with acute myocardial infarction in Baotou Central Hospital from April 2018 to March 2021 were randomly divided into control group and observation group,with 90 patients in each group.The control group was treated with percutaneous coronary intervention,and the observation group was treated with intra aortic balloon counterpulsation on the basis of the control group.The changes of serum growth differentiation factor-15(GDF-15),blood myocardial infarction,typeⅢcollagen,typeⅣcollagen and adhesin IOD values before and after treatment were compared.The changes of cardiac function and hemodynamic indexes before and after treatment were observed.Results After treatment,the three indexes of serum myocardial infarction in the two groups were significantly lower than those before treatment(P<0.05).After treatment,the levels of GDF-5,hemodynamic indexes and typeⅢcollagen,typeⅣcollagen and adhesin IOD in the two groups were significantly higher than those before treatment(P<0.05).After treatment,the level of LVED in the two groups decreased and LVEF increased(P<0.05).The above indexes changed more significamtly in the observation group(P<0.05).Conclusions IABP adjuvant treatment of AMI can significantly improve the cardiac function and hemodynamic indexes,increase the serum GDF-15,typeⅢcollagen,typeⅣcollagen and adhesin IOD values,and provide conditions for PCI revascularization.
