高频彩色多普勒超声对小儿肠套叠的诊断价值

Value of high frequency color Doppler ultrasound in the diagnosis of intussusception in children

目的探讨高频彩色多普勒超声对小儿肠套叠的诊断价值。方法选取2018年3月至2019年3月郑州儿童医院收治的80例肠套叠患儿。观察通过常规腹部X线检查、高频彩色多普勒超声检测方法确诊肠套叠患儿例数和图像显示情况,比较两种诊断的准确率、灵敏度和特异性。结果80例患儿中,高频与彩色多普勒超声检查图像呈现明显的"同心圆"及"假肾形",分别为44例(55.00%)和55例(68.75%);左侧腹部肠套叠7例(8.75%),右侧腹部肠套叠73例(91.25%)。高频彩色多普勒超声检测漏诊1例,准确率为98.75%(79/80),腹部X线片准确率为80.00%(64/80),高频彩色多普勒超声准确率显著高于腹部X线片诊断(P<0.05)。高频彩色多普勒超声检测小儿肠套叠的灵敏度为97.50%(78/80),特异性为96.25%(77/80);腹部X线片诊断灵敏度为85.00%(68/80),特异性为87.50%(70/80)。结论采用高频彩色多普勒超声的方法对小儿肠套叠进行诊断,准确率较高,可为患儿临床治疗提供可靠依据,该法具有高灵敏度和特异性,且无辐射、无创伤,操作方便快捷。

Objective To investigate the value of high frequency ultrasound combined with color Doppler ultrasound in the diagnosis of intussusception in children.Methods Eighty children with intussusception were admitted to Zhengzhou Children’s Hospital from March 20 to March 2019.The number of children with intussusception were diagnosed by routine abdominal X-ray examination and high frequency color Doppler ultrasound,and the accuracy,sensitivity and specificity of the two diagnosis methods were compared.Results Among the 80 children,the high frequency and color Doppler ultrasound images showed obvious"concentric circle"and"false kidney shape"in 44 case(55.00%)and 55 cases(68.75%)respectively;there were 7 children with left abdominal intussusception and 73 children with right abdominal intussusception,accounting for 8.75%and 91.25%respectively.The accuracy rate of high frequency color Doppler ultrasound was 98.75%(79/80)and 80.00%(64/80)respectively.The accuracy rate of high frequency color Doppler ultrasound was significantly higher than that of abdominal X-ray(P<0.05).The sensitivity and specificity of high frequency color Doppler ultrasound in the detection of intussusception in children were 97.50%(78/80),96.25%(77/80)and85.00%(68/80),87.50%(70/80)respectively.Conclusions High-frequency ultrasound combined with color Doppler ultrasound is used to diagnose the intussusception of children with high accuracy,which can provide a reliable basis for clinical treatment of children.The method has high sensitivity and specificity,and no radiation.It is non-invasive and easy to operate.
