埃索美拉唑治疗抗凝药物所致上消化道出血的效果

Effect of Esomeprazole on upper gastrointestinal hemorrhage caused by anticoagulants

目的分析埃索美拉唑治疗抗凝药物所致上消化道出血患者的应用价值。方法选取2017年9月至2019年1月在平顶山市第一人民医院治疗的78例上消化道出血患者,所有患者均为长期应用抗凝药物所致,将其分为两组,每组39例。对照组采取泮托拉唑治疗;观察组采取埃索美拉唑治疗。比较两组治疗总有效率、用药2、4、6 d患者的胃液pH值,随访期间再出血率。结果观察组患者治疗总有效率为89.74%(35/39),对照组治疗总有效率为71.79%(28/39),差异有统计学意义(P<0.05)。用药2、4、6 d,观察组患者的胃液pH值均明显高于对照组(P<0.05),随访期间,观察组再出血率为15.38%(6/39),对照组为38.46%(15/39),差异有统计学意义(χ~2=5.28,P<0.05)。结论对抗凝药物所致上消化道出血患者采取埃索美拉唑治疗可促进凝血块快速形成,提高胃液PH值,降低再出血率,治疗效果良好。

Objective To analyze the value of Esomeprazole in the treatment of upper gastrointestinal hemorrhage caused by anticoagulants.Methods Seventy-eight patients with upper gastrointestinal bleeding who were treated in our hospital from September 2017 to January 2019 were divided into two groups,with 39 cases in each group.The patients in the control group were treated with pantoprazole,and the patients in the observation group were treated with esomeprazole.The total effective rate,pH value of gastric juice at medication 2,4,6 days and rebleeding rate during follow-up were compared between the two groups.Results The total effective rate of the observation group was 89.74%(35/39),the total effective rate of the control group was 71.79%(28/39),the difference was significant(P<0.05).After 2,4,6 days of administration,the pH value of gastric juice in the observation group was significantly higher than that in the control group(P<0.05).During the follow-up period,the rebleeding rate was 15.38%(6/39)in the observation group and 38.46%(15/39)in the control group,the difference was significant(χ~2=5.28,P<0.05).Conclusions Esomeprazole can promote the rapid formation of clots in patients with upper gastrointestinal hemorrhage caused by anticoagulants,improve the pH value of gastric juice,reduce the rate of rebleeding,and the treatment effect is good.
