ACBT在呼吸衰竭患者拔管后HFNC序贯通气中的应用观察

Application of acbt in hfnc sequential ventilation after extubation in patients with respiratory failure

目的探讨主动呼吸循环技术在呼吸衰竭患者拔管后经鼻高流量氧疗序贯通气患者中的应用效果。方法将47例符合研究标准的患者随机分为对照组(24例)和实验组(23例)。对照组实施雾化、翻身、扣背等常规护理工作,实验组在常规护理基础上实施每日3次的ACBT。比较两组患者拔管72小时后的观察指标(首次排痰时间、呼吸音改善、血气分析结果、膈肌收缩距离、E-T指数及膈肌厚度等)。结果与对照组相比,实验组在首次排痰时间、呼吸音改善、血气分析结果、首次拔管后平均住院日、再次插管率、膈肌收缩距离及膈肌厚度明显优于对照组(P<0.05),X线改善、膈肌收缩时间差异无统计学意义(P>0.05)。结论ACBT可提高呼吸衰竭患者拔管后HFNC序贯通气中的自主排痰能力、呼吸肌运动功能,有效降低首次拔管平均住院日及再次插管率,可促进患者早期肺功能康复。

Objective To evaluate the effect of active cycle of breathing technology in respiratory technology in respiratory failure patients with respiratory failure after extubation for high flow nasal cannula.Methods Totally 47 cases of patients were randomly divided into control group(24 cases)and experimental group(23 cases).The control group was given blood gas analysis results,diaphragm contraction distance,E-T index and diaphragm thickness,etc.Results Compared with the control group,the experimental group The results of the first sputum discharge,the significantly better than those in the control group(P<0.05),There was no significant difference in X-line、diaphragm contraction time(P>0.05).Conclusion ACBT can improve the spontaneous sputum excretion ability and respiratory extubation and promote the early recovery of pulmonary function.
