右美托咪定辅助全身麻醉对老年结直肠癌患者术后早期认知功能的影响研究

Effect of dexmedetomidine assisted general anesthesia on early postoperative cognitive function in elderly patients with colorectal cancer

目的研究右美托咪定辅助全身麻醉用于老年结直肠癌患者术后早期认知功能及炎性因子的影响。方法2019年2月至2021年1月本科接诊老年结直肠癌病患82例,随机均分2组。术中观察组全身麻醉加用右美托咪定,对照组用等量生理盐水。对比MMSE(简易智力状态检查量表)评分、手术耗时和术中出血量、炎性因子(TNF-α、IL-6)等指标。结果针对MMSE评分,观察组术后第1 d和第3 d依次是(28.01±1.25)分、(29.34±0.41)分,比对照组(25.13±1.39)分、(26.29±2.36)分低,差异有统计学意义(P<0.05)。针对手术耗时与术中出血量,两组对比,差异无统计学意义(P>0.05)。针对TNF-α与IL-6水平,观察组术后第1 d依次是(101.02±15.28)pg/mL、(85.73±8.19)pg/mL,比对照组(124.69±17.21)pg/mL、(108.48±9.62)pg/mL低,差异有统计学意义(P<0.05)。结论于老年结直肠癌中运用右美托咪定辅助全身麻醉,利于术后认知功能的恢复,炎症因子水平的改善。

Objective To study the effects of dexmedetomidine assisted general anesthesia on early postoperative cognitive function and inflammatory factors in elderly patients with colorectal cancer.Methods From February 2019 to January 2021,82 elderly patients with colorectal cancer were randomly divided into two groups.Dexmedetomidine was added to general anesthesia in the study group,and the same amount of normal saline was used in the control group.The MMSE score,operation time,blood loss and inflammatory factors(TNF-α,IL-6)were compared.Results The MMSE scores of the study group were(28.01±1.25)and(29.34±2.01)on the first day and the third day after operation,which were lower than(25.13±1.39)and(26.29±2.36)in the control group,the difference was statistically significant(P<0.05).There was no significant difference in operation time and intraoperative blood loss between the two groups(P>0.05).The levels of TNF-αand IL-6 in the study group were(101.02±15.28)pg/mL,(85.73±8.19)pg/ml on the first day after operation,which were lower than(124.69±17.21)pg/mL,(108.48±9.62)pg/mL in the control group,the difference was statistically significant(P<0.05).Conclusion Dexmedetomidine assisted general anesthesia in elderly patients with colorectal cancer is beneficial to the recovery of postoperative cognitive function and the improvement of inflammatory factors.
