成人体外膜肺氧合支持病人的营养支持治疗研究进展

Progress of nutritional therapy in adult patients supported by extracorporeal membrane oxygenation

体外膜肺氧合(ECMO)主要用于治疗严重心功能和呼吸功能衰竭的危重症病人,以纠正其低氧血症,维持血流动力学稳定。接受ECMO治疗的病人在营养素需求、机体代谢、液体控制等方面与普通病房病人存在很大差异,这类病人往往存在较高的营养风险,然而目前暂无ECMO支持病人的临床营养治疗共识或指南。本文拟对ECMO支持病人的营养支持治疗相关进展进行文献综述,拟为临床该类病人的营养治疗提供最新的治疗依据。

Extracorporeal membrane oxygenation(ECMO) is mainly used to treat in critical patients with severe heart cardiac and/or respiratory failure to correct hypoxemia, improve oxygen metabolism and maintain hemodynamic stability. Patients receiving ECMO treatment differ greatly from patients in general wards in terms of nutrient requirements, body metabolism, fluid control, etc. ECMO patients are seriously ill and have severe stress reactions. They are quite different from ordinary patients in terms of nutritional metabolism, nutrients needs and hydration. Such patients are often have at high nutritional risks. However, there is no consensus or guidelines for clinical nursing nutritional treatment of patients supported by ECMO at present. This paper intends to review the literature on the progress of nutritional support treatment in ECMO supported patients, and in order to provide the latest treatment basis for the nutritional treatment of such patients.
