某院2 051例患者呼吸道病原体九联检结果分析 被引量:4

摘要 目的分析本院呼吸道病原体九联检检出情况及季节分布情况。方法收集2018年9月—2019年9月在曲靖市第二人民医院进行呼吸道病原体九联检的2051例患者血清,采用间接免疫荧光法(IFA)检测血清中9种呼吸道感染病原体〔包括嗜肺军团菌血清1型(LP1)、肺炎支原体(MP)、Q热立克次体(QFR)、肺炎衣原体(CPn)、腺病毒(ADV)、呼吸道合胞病毒(RSV)、甲型流感病毒(IFVA)、乙型流感病毒(IFVB)和副流感病毒(PIVs)〕免疫球蛋白M(IgM)抗体,分析不同性别、不同季节的呼吸道病原体检出情况以及单一感染与复合感染情况。结果共纳入2051份患者血清样本,IgM抗体阳性393份,阳性率为19.16%。其中男性阳性率15.29%(183/1197),女性阳性率24.59%(210/854)。呼吸道感染以单一病原体感染为主,阳性率为17.16%(352/2051);MP阳性率最高〔10.48%(215/2051)〕,其次为QFR〔1.51%(31/2051)〕,IFVA最低〔0.20%(4/2051)〕。复合感染阳性率为2.00%(41/2051),MP合并IFVB阳性率最高〔0.68%(14/2051)〕。呼吸道感染以春季发病率最高(26.99%),其次为秋季发病(24.64%),冬季发病(19.87%),夏季发病率最低(6.23%)。结论本院呼吸道感染患者多为单一病原体感染,以MP为主;发病率与季节相关,春、秋两季多发。呼吸道病原体九联检能同时检测多种病原体IgM抗体,为患者的诊疗和抗菌药物的合理使用提供依据。 Objective Using nine-joint inspection,to analyze the detection and seasonal distribution of respiratory pathogens in our Hospital.Methods The serum samples of 2051 patients under the nine-joint inspection of respiratory pathogens in the Second People’s Hospital of Qujing from September 2018 to September 2019 were collected.The immunoglobulin M(IgM)antibodies against nine pathogens of respiratory tract infection[including Legionella pneumophila serotype 1(LP1),Mycoplasma pneumoniae(MP),Q-rickettsia(QFR),Chlamydia pneumoniae(CPn),adenovirus(ADV),respiratory syncytial virus(RSV),influenza a virus(IFVA),influenza b virus(IFVB)and para-influenza virus(PIVs)]were detected by indirect immunofluorescence assay(IFA).Furthermore,the physical examination of respiratory pathogens,single infection and multiple infection in different seasons and genders were analyzed.Results A total of 2051 serum samples were included,393 of which were IgM antibody positive,with a positive rate of 19.16%.The positive rate of male was 15.29%(183/1197),and the positive rate of female was 24.59%(210/854).Respiratory tract infection was mainly caused by a single pathogen,and the positive rate was 17.16%(352/2051).The positive rate of MP was the highest(10.48%(215/2051)),followed by QFR(1.51%(31/2051)),and that of IFVA was the lowest(0.20%(4/2051)).The positive rate of mixed infection was 2.00%(41/2051),and that of MP combined with IFVB was the highest(0.68%(14/2051)).The incidence of respiratory tract infection was the highest in spring(26.99%),followed by autumn(24.64%),winter(19.87%),and summer(6.24%).Conclusions In our hospital,most of the patients with respiratory tract infection are infected by single pathogen,mainly by MP;the incidence is related to season and is higher in spring and autumn.Nine-joint inspection of respiratory pathogens can simultaneously detect IgM antibodies of various pathogens,providing basis for diagnosis and treatment of patients and rational use of antibiotics.
