三维超声评估不同分娩方式对初产妇盆底结构的影响

Evaluation of different delivery mode on the pelvic floor structure of primipara by Three-dimensional Ultrasound

目的:使用盆底三维超声评估不同分娩方式对初产妇盆底结构的影响。方法:选取2017年10月-2020年7月在本院行三维盆底超声检查的经阴道分娩6~8周初产妇75例为观察组,另选同期在本院剖宫产分娩的初产妇作为对照组75例,记录并分析两组产妇在静息状态和Valsalva状态下盆底的形态结构参数。结果:经观察组在膀胱颈距离耻骨联合后下缘水平线距离小于对照组,在膀胱颈移动度、膀胱尿道后角、尿道旋转角、肛提肌裂孔面积均大于对照组(P<0.05)。经观察组盆腔器官脱垂及压力性尿失禁发生率高于对照组(P<0.05)。结论:经阴道分娩者较剖宫产分娩者在产后6~8周更容易发生盆底障碍性疾病,盆底超声可以很好地观察盆底的结构功能,从而为临床诊疗提供帮助。

Objective:To evaluate the effects of different delivery mode on the pelvic floor structure of primiparas by Three-dimensional Ultrasound examination.Methods:75 primiparas with vaginal delivery who underwent three-dimensional pelvic floor ultrasound examination in our hospital for 6-8 weeks after delivery from October 2017 to July 2020 were selected as study group,and another 75 primiparas with cesarean delivery in our hospital during the same period were selected as control group.Record and analyze the morphological and structural parameters of the pelvic floor in the resting state and Valsalva state of the two groups.Results:The distance from bladder neck to lower margin of pubic symphysis in study group was statistically smaller than that in controls,the bladder mobility,posterior vesicourethral angle,urethral rotation angle,and the area of levator hiatus in study group were larger than those in controls,and the difference was statistically significant(P<0.05).The incidence of Pelvic organ prolapse and Stress urinary incontinence in the vaginal delivery group was higher than that in the cesarean section group,the difference was significant(P<0.05).Conclusion:6-8 weeks after delivery,vaginal delivery is more prone to pelvic floor disorders than cesarean section.pelvic floor ultrasound can be a good observation of pelvic floor structure and function for clinical diagnosis and treatment.
