宫腔镜治疗宫腔占位伴异常出血疾病88例临床分析

Clinical Analysis of Intrauterine Occupational with Abnormal Uterine Bleeding Disease treated by Hysteroscopy in 88 Cases

目的:探讨宫腔镜电切术诊断和治疗宫腔占位合并异常子宫出血疾病的临床疗效以及对诊治此类疾病的重要价值。方法:选取2017年5月至2019年6月宫腔占位合并有异常子宫出血患者共88例,均行宫腔镜电切术,术后比较宫腔镜诊断与病理诊断的符合率,评价临床疗效和价值。结果:88例患者中,子宫内膜息肉45例,子宫粘膜下肌瘤33例,子宫内膜癌2例,子宫肉瘤1例,妊娠相关疾病4例,宫内异物残留3例。宫腔镜诊断的符合率高于临床诊断(P<0.05)。随访至术后6个月并评价治疗效果,术后3个月时较随访1个月时经期子宫出血量改善(P<0.05),术后6个月时较随访1个月时子宫出血量、超声检查宫腔占位也减小或至消失(P<0.05)。结论:宫腔镜电切术以其直观,准确,微创等优点在诊断和治疗宫腔占位伴异常出血疾病中具有重要的临床价值。

Objective:To discuss the clinical efficacy and important significance of intrauterine occupational with abnormal uterine bleeding disease treated by hysteroscopy electrocision.Methods:Select 88 case of intrauterine occupational with abnormal uterine bleeding from May 2017 to June 2019,treated by hysteroscopy electrocision.Compare the postoperation hysteroscopic and pathological diagnostic coincidence rate,evaluate the clinical efficacy and significance.Result:88 case include 45 case of endometrial polyp,33 case of submucous uterine myoma,2 case of endometrial cancer,1 case of uterine sarcoma,4 case of gestation-related diseases,3 case of intrauterine foreign matter.hysteroscopic diagnostic coincidence rate is higher than clinic diagnostic(P<0.05).The patients were followed up to 6 months after operation and the therapeutic effect was evaluated.The amount of uterine bleeding during menstruation was significantly improved at 3 months after operation(P<0.05),and the amount of uterine bleeding and uterine space occupied by ultrasound were significantly decreased or disappeared at 6 months after operation(P<0.05).The operation result treated by hysteroscopy is good.Conclusion:Hysteroscopy electrocision as a visual,accurate and minimally invasive treatment for intrauterine occupational with abnormal uterine bleeding disease has important clinic significance.
