头颅MRI病灶“心型”演变成“Y”型的双侧延髓内侧梗死1例报道并文献资料分析 被引量:1

摘要 目的:通过双侧延髓内侧梗死(BMI)临床表现、影像学改变重新认识该疾病。方法:总结该患者临床资料并文献资料分析。结果:该患者病情迅速进展至四肢瘫痪、浅感觉障碍、真性球麻痹、呼吸肌受累,影像学上病灶较前扩大,最终死亡。结论:1,由于BMI的临床表现多样,临床医生结合掌握患者病情特点逐步推理可以减少误诊几率。2,头颅MRI检查,尤其是DWI(高信号)-ADC(低信号)序列对BMI的早期诊断有很大的帮助,病灶由"心型"演变成"Y"型提示患者病情加重、预后不良。3,双侧延髓内侧梗死病灶呈"Y"型实际上是脊髓前动脉闭塞综合征变种,区别在于:梗死部位不同,临床表现不同。4,该疾病进展凶险、预后不良,患者疾病在进展过程中,影像学DWI-FLAIE不匹配,治疗上追求个体化原则,根据最新2018缺血性卒中指南规定可以尝试在24小时溶栓,应该大胆尝试静脉溶栓,或者直接椎动脉机械取栓治疗搏取生存机会。
