外周血miR-92a、miR-375及miR-760对结直肠癌不同TNM分级的诊断预测价值 被引量:1

Diagnostic and predictive value of miR-92a,miR-375 and miR-760 in peripheral blood for different TNM staging of colorectal cancer

摘要 目的 探讨外周血miR-92a、miR-375及miR-760对结直肠癌不同TNM分级的诊断价值。方法 选取2018年1月至2021年1月运城市中心医院收治的70例结直肠癌患者,提取其血清总RNA行反转录,并采用实时荧光定量聚合酶链反应(quantitative real-time fluorescence polymerase chain reaction, RT-qPCR)分析不同TNM分期外周血miR-92a、miR-375及miR-760的表达水平。采用Pearson相关性分析miR-92a、miR-375及miR-760表达水平与TNM分期的相关性,采用ROC曲线分析3种miRNAs在不同TNM分期的诊断价值。结果 miR-92a在结直肠癌TNM各分期中的表达呈上升趋势,在不同期的分布差异有统计学意义(P <0.05)。miR-760在结直肠癌TNM各分期中的表达呈下降趋势,在不同期的分布差异有统计学意义(P <0.05)。miR-375在TNM各分期中表达水平呈下降趋势,在不同期的分布差异有统计学意义(P<0.05)。miR-92a、miR-375及miR-760表达水平与性别和年龄均无相关性(P> 0.05),miR-92a表达水平与结直肠癌TNM分期呈正相关(r=0.683,P <0.01),miR-375、miR-760表达水平与结直肠癌TNM分期呈负相关(r=-0.472,-0.684,P <0.01)。miR-92a、miR-375及miR-760鉴别结直肠癌TNM IV期和TNMⅠ~Ⅲ期的AUC分别为0.926(95%CI:0.856~0.997),0.804(95%CI:0.622~0.985)和0.836(95%CI:0.733~0.938),灵敏度分别为100.00%,63.64%和72.73%,特异性分别为73.58%,86.79%和76.56%。结论 miR-92a、miR-375及miR-760可以鉴别结直肠癌不同TNM分期,有望应用于其预后判断。 Objective To explore the diagnostic value of miR-92a,miR-375 and miR-760 in peripheral blood for TNM staging of colorectal cancer.Methods A total of 70 patients with colorectal cancer admitted to Yuncheng Central Hospital from January 2018 to January 2021 were selected,and the serum total RNA of the patients was extracted for reverse transcription.Real-time fluorescence quantitative polymerase chain reaction(RT-qPCR)was used to analyze the expression levels of miR-92a,miR-375 and miR-760 in peripheral blood of the patients with different TNM staging.Pearson correlation was used to analyze the expression correlation between miR-92a,miR-375,miR-760 and TNM staging.ROC curve was used to analyze the diagnostic value of the three miRNAs in different TNM staging.Results The expression of miR-92a in different TNM staging of colorectal cancer showed an upward trend,and the distribution difference was statistically significant at different periods(P<0.05).The expression of miR-760 in different TNM staging of colorectal cancer showed a downward trend,and the distribution difference was statistically significant(P<0.05).The expression level of miR-375 showed a downward trend in different TNM stagings,and the distribution difference was statistically significant(P<0.05).The expression levels of miR-92a,miR-375 and miR-760 were not correlated with gender and age(both P>0.05),while the expression levels of miR-92a were positively correlated with TNM staging of colorectal cancer(r=0.683,P<0.01).The expression levels of miR-375 and miR-760 were negatively correlated with TNM staging of colorectal cancer(r=-0.472,-0.684,P<0.01).The AUCs of miR-92a,miR-375 and miR-760 in differentiating TNM IV staging and TNM I~III were 0.926(95%CI:0.856-0.997),0.804(95%CI:0.622-0.985)and 0.836(95%CI:0.733-0.938),respectively,and the sensitivity was 100.00%,63.64%and 72.73%,the specificity was 73.58%,86.79%and 76.56%,respectively.Conclusions miR-92a,miR-375 and miR-760 can differentiate different TNM staging of colorectal cancer,and are expected to be used in prognosis judgment.
