改良腮腺切除术对腮腺肿瘤患者预后的影响

Impact of modified parotidectomy on the prognosis in patients with parotid tumor

目的:探讨改良腮腺切除术对腮腺肿瘤患者预后的影响。方法:选取2017年1月至2019年1月本院收治的腮腺肿瘤患者60例,随机分为改良组与对照组,每组30例。对照组予以传统腮腺切除手术治疗,改良组采用改良腮腺切除术治疗。比较两组围手术期指标(出血量、住院时间、手术时间)及术后并发症(切口感染、口干、面瘫、味觉性出汗综合征)发生率,记录两组患者术前、术后6个月的生活质量评分及随访1年期间的复发率。结果:两组出血量、手术时间比较,差异无统计学意义(均P>0.05)。改良组住院时间短于对照组,并发症总发生率低于对照组(均P<0.05)。术后6个月,两组患者生活质量各项评分均较治疗前上升,且改良组优于对照组(均P<0.05)。随访1年内,改良组无复发,对照组复发率为6.7%,两组比较差异无统计学意义(P>0.05)。结论:改良腮腺切除术治疗腮腺肿瘤效果更佳,能减少住院时间,改善生活质量,降低术后并发症发生率。

Objective:To investigate the impact of modified parotidectomy on the prognosis of patients with parotid tumor.Methods:A total of 60 patients with parotid tumor treated in our hospital between January 2017 and June 2019 were included in this study and randomly divided into the study group and control group(n=30 each).The control group was treated with traditional parotidectomy and the study group with modified parotidectomy.The perioperative indexes(blood loss,length of hospital stay,and operation time)and postoperative complications(wound infection,dry mouth,facial paralysis,and gustatory sweating syndrome)were compared between the two groups.The quality of life at baseline and at 6 months after operation,as well as the recurrence rate during the one-year follow-up in the both groups were recorded.Results:There was no significant difference in blood loss and operation time between the two groups(P>0.05).The study group experienced shorter length of hospital stay and lower overall rate of complications compared with the control group(bothP<0.05).At 6 months after operation,the both groups showed higher sub-scores of life quality compared with baseline,and the sub-scores were higher in the study group compared with the control group(allP<0.05).During the one-year follow-up,the recurrence rate was zero in the study group and 6.7%in the control group,but there was no statistical difference between the two group(P>0.05).Conclusion:Modified parotidectomy is more effective in treatment of parotid tumor,with shorter lenght of hospital stay,improved quality of life,and lower rate of postoperative complications.
