缝线法和部分睫毛切除法治疗小儿下睑倒睫的效果及预后观察

Efficacy and outcome of suturing vs partial eyelash removal for treatment of lower eyelid trichiasis in children

目的:探究缝线法和部分睫毛切除法治疗小儿下睑倒睫的效果及预后观察。方法:回顾性分析2016年12月~2018年12月于我院接受治疗的74例下睑倒睫患儿的临床资料,根据治疗方式不同分组,其中对照组(缝线法,患眼60只)40例,观察组(部分睫毛切除法,患眼57只)34例。比较两组术前、拆线后的患儿家属美观满意度,并记录两组患儿随访6个月后的临床疗效、并发症与复发率情况。结果:拆线后,观察组患儿家属总满意度为88.2%,对照组77.5%,但两组之间比较无统计学意义(P均>0.05);随访6个月,观察组的临床治疗总有效率为89.5%,显著高于对照组的75.0%(P<0.05);随访6个月,两组患儿并发症发生率无统计学意义(P>0.05);随访6个月,观察组的复发率为5.9%,显著低于对照组的22.5%(P<0.05)。结论:缝线法和部分睫毛切除法均可用于小儿下睑倒睫,且患者术后满意度较高,但其中部分睫毛切除法治疗效果更好,而缝线术复发率更高。

Objective:To investigate the efficacy and outcome of suturing vs partial eyelash removal for treatment of lower eyelid trichiasis in children.Methods:A retrospective analysis was performed on the clinical data of 74 children with lower eyelid trichiasis treated in our hospital between December 2016 and December2018.According to treatment options,the patients were divided into the control group to receive eyelid suturing(n=40,comprising 60 affected eyes)and the study group to receive partial eyelash removal(n=34,comprising 57 affected eyes).The two groups were compared for parent satisfaction about aesthetics,efficacy,complications and recurrence of the condition as recorded at 6 months of follow up.Results:After removal of the stitches,the overall parent satisfaction in the study group was 88.2%vs 77.5%in the control group,but the difference was not statistically significant(allP>0.05).At 6 months of follow-up,the overall effective rate of clinical treatment in study group was significantly higher than that in control group(89.5%vs 75.0%,P<0.05);there was no significant difference in the incidence of complications between the two groups(P>0.05);recurrence was noted in 5.9%of children in the study group,which was significantly lower than the rate of 22.5%in the control group(P<0.05).Conclusion:Both suturing and partial eyelash removal are useful for treatment of lower eyelid trichiasis in children with favorable parent satisfaction.The treatment outcomes can be better with partial eyelash removal.On the other hand,the suturing method may be associated with higher chance of postoperative recurrence.
