注射用奥沙利铂联合B超引导下经皮射频消融术对原发性肝癌患者肝功能及生存率的影响

Effects of intravenous oxaliplatin combined with B-ultrasound guided percutaneous radiofrequency ablation on liver function and survival rate in patients with primary hepatic carcinoma

目的:探讨注射用奥沙利铂联合B超引导下经皮射频消融术对原发性肝癌(PHC)患者肝功能及生存率的影响。方法:选取2016年9月~2018年3月我院PHC患者68例,按治疗方案分观察组(n=34)、对照组(n=34)。对照组予以B超引导下经皮射频消融术治疗,观察组予以注射用奥沙利铂联合B超引导下经皮射频消融术治疗。比较两组疗效、治疗前后肝功能[血清谷氨酸氨基转移酶(ALT)、总胆红素(TBIL)、谷氨酰转肽酶(GGT)水平]、肝功能(Child)分级,随访1年统计两组生存率。结果:观察组控制率97.06%(33/34)较对照组76.47%(28/34)高(P<0.05);治疗后两组血清ALT、TBIL、GGT水平均降低,且观察组较对照组低(P<0.05);治疗后观察组Child分级优于对照组(P<0.05);观察组生存率96.77%(30/31)较对照组73.33%(22/30)高(P<0.05)。结论:注射用奥沙利铂联合B超引导下经皮射频消融术治疗PHC疗效显著,能有效改善肝功能,提高生存率。

Objective:To investigate the effects of intravenous oxaliplatin combined with B ultrasoundguided radiofrequency ablation on liver function and survival rate in patients with primary hepatic carcinoma(PHC).Methods:A total of 68 PHC patients in our hospital between September 2016 and March 2018 were enrolled,and assigned to the study observation group(n=34)and the control group(n=34)according to the treatment protocol.The control group was treated with B-ultrasound guided percutaneous radiofrequency ablation alone,while the study group was treated with intravensous oxaliplatin combined with B-ultrasound guided percutaneous radiofrequency ablation.The two groups were compared for the treatment efficacy,liver function before and after treatment[serum glutamate aminotransferase(ALT),total bilirubin(TBIL),and gammaglutamyl transpeptidase(GGT)levels],liver function(Child)classification,and 1-year survival rate at followup.Results:The tumor control rate in the study group was 97.06%(33/34),which was higher than that in control group[76.47%(28/34)](P<0.05).After treatment,the serum ALT,TBIL and GGT levels were reduced in both groups,and the study group showed even lower levels of these indicators than the control group(P<0.05).The Child classification was more favorable in the study group compared with the control group(P<0.05).The survival rate in the study group was 96.77%(30/31),which was higher than that in the control group[73.33%(22/30)](P<0.05).Conclusion:Intravenous Oxaliplatin combined with B-ultrasound guided percutaneous radiofrequency ablation is effective in the treatment of PHC,resulting in effectively improved liver function and survival rate.
