去氧肾上腺素稳定老年患者全麻诱导期血流动力学的研究

Study on hemodynamics during induction of general anesthesia in elderly patients with low-dose phenylephrine

目的:研究小剂量去氧肾上腺素静注预防老年患者全身麻醉诱导期低血压的应用效果。方法:选择2018年10月至2019年4月期间在我院接受全身麻醉的50例老年前列腺增生患者作为观察对象,随机分成对照组、研究组,各25例。对照组静注生理盐水,研究组静注小剂量去氧肾上腺素。记录两组患者麻醉诱导前后不同时间段的血压、心率,输入液体量,额外使用血管活性药及术后不良事件发生率。结果:研究组T2、T3、T4时S组平均动脉压高于D组(P<0.05),低血压发生率S组血压低于D组(P<0.05),两组患者诱导期液体入量S组低于D组(P<0.05),额外血管活性药使用率S组低于D组(P<0.05)。结论:预注小剂量去氧肾上腺素使老年患者全麻诱导更平稳。

Objective:To study the effect of low-dose phenylephrine intravenous injection on prevention of hypotension in induction of general anesthesia in elderly patients.Methods:50 elderly patients with benign prostatic hyperplasia who underwent general anesthesia in our hospital from October 20 to March 2019 were selected as subjects and were randomly divided into control group and study group,25 cases each.The control group received intravenous saline and the study group received a small dose of phenylephrine.The blood pressure and heart rate of the two groups before and after anesthesia induction were recorded.Postoperative adverse event rate and Input liquid volume were recorded too.Results:The mean arterial pressure of the S group was higher than that of the D group in the study group at T2,T3 and T4(P<0.05).The incidence of hypotension is lower in group S than in group D(P<0.05).Intravenous fluid intake in group S was lower than group D(P<0.05).Additional vasoactive drug use rate in group S was lower than group D(P<0.05).Conclusion:Pre-filling lowdose phenylephrine makes the induction of general anesthesia more stable in elderly patients.
