两种手术方法治疗唇裂鼻畸形的疗效比较

Two surgical options for cleft lip deformity:comparison of the treatment efficacy

目的:比较两种手术方法治疗唇裂鼻畸形的治疗效果。方法:收集2016年12月至2017年12月在我院手术治疗的单侧唇裂鼻畸形患者124例,其中行翼软骨环状结构修复及整体旋转复位术患者62例为观察组,行开放式唇裂鼻畸形整形术患者62例为对照组,比较两组的修复效果及并发症发生情况。结果:观察组和对照组术后外观优良率分别为96.77%(60/62)、95.16%(59/62),组间比较差异无统计学意义(P=0.794)。两组患者术后患侧鼻孔纵径至中线距离、患侧鼻翼外侧至中线距离与术前比较,差异均有统计学意义(均P<0.001)。术后两组患者患侧与健侧鼻孔纵径至中线距离、患侧与健侧鼻翼外侧至中线距离组内比较,差异均无统计学意义(均P=1.000)。观察组与对照组并发症发生率分别为4.84%(3/62)、6.45%(4/62),差异无统计学意义(P>0.05)。结论:两种手术方法治疗单侧唇裂鼻畸形临床疗效基本接近。

Objective:To compare the treatment efficacy with of two surgical options for cleft lip nasal deformity(CLND).Methods:Included in this study were a total of 124 patients with unilateral CLND who underwent surgery in our hospital between December 2016 and December 2017.Among them,62 patients who underwent repair of alar cartilaginous ring and Millard’s surgery were assigned to the study group,and another 62 who underwent open plastic surgery for CLND were assigned to the control group.The outcomes of repair and the complications were compared between the two groups.Results:The rate of cosmetic excellence in the study group and the control group were 96.77%(60/62)and 95.16%(59/62),respectively,with no significant difference between the groups(P=0.794).After surgery,there were significant differences in the distance from the longitudinal axis of ipsilateral nostril to the midline and the distance from lateral side of ipsilateral alar to the midline within each group compared with baseline(bothP<0.001).There was no significant difference in the distance from longitudinal axis of nostril to the midline between the ipsilateral and contralateral sides,or in the distance from lateral side of alar to the midline between ipsilateral and contralateral sides,within either group(allP=1.000).The complication rates in the study group and the control group were 4.84%(3/62)and 6.45%(4/62),respectively,with no significant difference(P>0.05).Conclusion:The clinical efficacy of these two surgical methods appears overall comparable in treatment of unilateral cleft lip nasal deformity.
