单中心连续200例单操作孔胸腔镜治疗早中期非小细胞肺癌临床体会

Single-port thoracoscopic surgery in treatment of early/mid-stage non-small cell lung cancer: A single-center experience with 200 consecutive cases

目的:探讨单操作孔胸腔镜治疗早中期非小细胞肺癌安全性、可行性及有效性。方法:回顾性分析2017年1月1日至2018年12月31日在高州市人民医院行单操作孔胸腔镜肺癌根治手术的200例早中期非小细胞肺癌患者的临床资料。结果:术中中转开胸19例(9.5%);随着技术开展,手术时间[平均(266.79±50.64)min]、术中出血[平均(191.55±101.99)mL]呈现线性下降趋势;淋巴结清扫枚数稳定上升[平均(17±5)枚];主要并发症发生率降低,术后住院天数下降[平均(6.0±1.7)d]。结论:经过严格学习曲线学习,有序开展单孔胸腔镜肺癌根治术安全可行,短期治疗疗效好。

Objective:To investigate the safety,feasibility and efficacy of single-port thoracoscopic surgery in the treatment of early-to-mid-stage non-small cell lung cancer(NSCLC).Methods:The clinical data of 200 patients with early-to-mid-stage NSCLC,who underwent single-port thoracoscopic lung cancer radical surgery in Gaozhou Municipal People’s Hospital between January 1,2017 and December 31,2018,were retrospectively analyzed.Results:Nineteen cases(9.5%)were converted to open chest surgery during the operation.Along with the use of thoracoscopic techniques,the operation time[mean(266.79±50.64)]min and intraoperative blood loss[mean(191.55±101.99)mL]showed a trend of linear decline;the number of lymph node dissections increased steadily[mean(17±5)lymph nodes];the incidence of major complications and the postoperative length of hospital stay decreased[mean(6.0±1.7)d].Conclusion:After rigorous training according to the learning curve,proper use of single-port thoracoscopic radical surgery for lung cancer is safe and feasible,with satisfactory short-term treatment efficacy.
