西宁地区脑卒中后睡眠障碍相关因素分析

Analysis of the Related Factors on Sleep Disorder after Stroke in Xining Area

目的:探讨西宁地区脑卒中后睡眠障碍的影响因素。方法:采用阿森斯失眠量表和美国国立卫生院神经功能缺损评分对我科住院的急性脑卒中60例患者进行测评,按测评结果分为睡眠障碍组与非睡眠障碍组,居住地区海拔高度2260m^2800m。结果:60例急性脑卒中患者,伴睡眠障碍者27例,发生率45.0%;脑卒中后睡眠障碍组患者在年龄、性别、饮酒、吸烟、神经功能缺损评分、卒中性质、部位、分布、血脂、血糖、神经元特异性烯醇化酶及血红蛋白与非睡眠障碍组比较,差异无统计学意义(P>0.05),而高血压、糖尿病、打鼾史及高同型半胱氨酸与非睡眠障碍组比较,差异有统计学意义(P<0.05)。结论:西宁地区脑卒中后睡眠障碍也是常见的临床症状,其与既往病史高血压、糖尿病、打鼾及高同型半胱氨酸有关,及时发现相关危险因素,予以积极干预,有利于改善患者的睡眠质量,加快神经功能的恢复。

Objective:This paper is aimed to explore the influencing factors of sleep disorder after stroke in Xining area.Methods:60 patients with acute stroke in our department were assessed with the Ascension Insomnia Scale(ALS)and the National Institutes of Health Stroke Scale(NIHSS).According to the Evaluation results,all patients were divided into sleep disorder group and non-sleep disorder group with the altitude of the residential area of 2260 m^2800 m.Results:Among the 60 patients with acute stroke,27 of them have sleep disorder,whose incidence rate is 45.0%.There were no significant differences between sleep disorder group and non-sleep disorder group in age,sex,alcohol consumption,smoking,NIHSS score,stroke nature,location,distribution,blood fat,glu,neuron specific enolase(NSE)and hemoglobin(HB)(P>0.05).However,there were significant differences in hypertension,diabetes,snoring history and high homocysteine(HCY)in each group(P<0.05).Conclusion:Sleeping disorder after stroke is a common clinical symptom in Xining area.It is related to hypertension,diabetes,snoring and homocysteine in the past.It is helpful to improve the sleep quality of patients and accelerate the recovery of nervous function by finding out the related risk factors and intervene actively in time.
